摘  要:目的研究Philips Brilliance i CT扫描仪(i CT)双低扫描技术(低辐射剂量、低浓度造影剂)在头颈部多排螺旋CT血管成像(MSCTA)的临床应用价值。方法将医院拟诊头颈部血管病变、最终按要求完成MSCTA查的患者66例,按不同管电压及对比剂浓度平均分为2组:A组:管电压100 k V,对比剂(碘佛醇)320 mg I/ml;B组:管电压120 k V,对比剂(碘海醇)350mg I/ml。所有患者对比剂用量均为50 ml,注射速率4.5 ml/s。使用飞利浦Bolus Tracking技术采集图像,由两名医师采用4级评分法对所有CTA图像进行质量评估,并比较两组的对比噪声比(CNR)、有效辐射剂量(ED)及碘摄入量。结果所有患者头颈部动脉及其分支均显示良好,两组病例头颈部血管强化程度和CNR无统计学差异(P>0.05);A组有效辐射剂量和碘摄入量低于B组(P<0.05)。结论 i CT在低辐射剂量(管电压100 k V)、低浓度造影剂(320 mg I/ml)下行头颈部MSCTA检查,可获得与高千伏、高浓度碘对比剂同样图像质量,完全满足临床诊断需要,且减少患者的有效辐射剂量和碘摄入量,具有较高临床应用价值。Objective To study the clinical application value of dual-low scanning technology (low radiation dose and low concentration of contrast agent) of Philips Brilliance iCT scanner (iCT) to head and neck multi-slice CT angiography (MSCTA). Methods A total of 66 patients with head and neck vascular lesions to receive MSCTA in our hospital were divided into two groups according to the different tube voltage and concentration of contrast agent: for group A, the tube voltage was 100 kV and contrastagent (ioverol) was 320 mgl/ml; for group B, the tube voltage was 120 kV and contrast agent (Iohexol) was 350 mgl/ml. For all the patients, the dosage of contrast agent was 50 ml and the injection rate was 4.5 ml/s. PHILPS Bolus Tracking technology was adopted for image acquisition, and evaluation on all CTA images was made by two doctors by using four-level scoring method. The contrast/ noise ratio (CNR), effective radiation dose ("effective dose" ,ED), and iodine intake between the two groups were compared and analyzed. Results The head and neck arteries and their branches of all patients showed good, and there were no statistically significant difference in head and neck vascular enhancement and CNR between the two groups. The ED and iodine intake in group A were lower than those in group B(P 〉 0.05). Conclusion iCT dual-low scanning technology (tube voltage: 100 kV; low concentration of contrast agent (320 mgI/ml)) in head and neck MSCTA can achieve high-quality image like High KV and high-concentration iodine contrast agent, fully meeting the needs of clinical diagnosis; it can also reduce the patient's ED and high iodine intake, so it has high clinical application value.
