D6单囊胚新鲜周期移植价值分析

摘　　要：目的分析比较新鲜周期第6d(D6)与新鲜周期第5d(D5)、解冻周期D6的单囊胚移植对妊娠结局的影响。方法回顾性分析2014年6月1日-2016年5月31日在我院生殖中心予常规IVF/ICSI-ET助孕治疗后行单囊胚新鲜/解冻移植患者。按囊胚形成天数以及周期类型分组。新鲜D6组:新鲜周期D6单囊胚移植,181周期;新鲜D5组:新鲜周期D5单囊胚移植,298周期;解冻D6组:解冻周期D6单囊胚移植,461个周期。分别比较新鲜D6组与新鲜D5组、新鲜D6组与解冻D6组的妊娠结局。结果新鲜D6组平均年龄显著低于解冻D6组,差异有统计学意义(P<0.001);其他一般情况组间均无统计学差异(P>0.05)。新鲜D6组较新鲜D5组HCG阳性率、临床妊娠率和胚胎种植率降低(69.61%vs 73.49%,60.56%vs 64.77%,61.88%vs 65.10%),早期流产率升高(5.50%vs 3.11%),但两组差异均无统计学意义(P>0.05)。新鲜D6组HCG阳性率、临床妊娠率和胚胎种植率与解冻D6组相比无显著性差异(69.61%vs 68.33%,60.56%vs 59.87%,61.88%vs 60.95%,P>0.05);新鲜D6组早期流产率低于解冻D6组(5.50%vs 9.06%),但两者差异无统计学意义(P>0.05)。结论 D6单囊胚移植的临床妊娠结局可能不如D5囊胚好。D6单囊胚新鲜移植有和其解冻移植类似甚至可能更好的临床妊娠结局。Objective To analyse and compare clinical pregnancy outcomes of fresh day 6（D6） and fresh day 5（D5）,vitrifiedwarmed D6 single blastocyst transfer cycles. Methods A retrospective study was conducted to assess the clinical pregnancy outcomes of fresh and frozen-thawed single blastocyst transfer cycles including 181 fresh D6 cycles,298 fresh D5 cycles and 461frozen-thawed D6 cycles in our center between June 2014 and May 2016.Results：The age in fresh D6 group was significantly lower than that in frozen-thawed D6 group（P〈0.001）;Other general situation had no statistical difference in these groups（P〈0.05）.Compared with fresh D5 group,HCG positive rate,clinical pregnancy rate and implantation rate in fresh D6 group decreased,early abortion rate increased but they had no significant differences（69.61% vs 73.49%,60.56% vs 64.77%,61.88% vs 65.10%,5.50%vs 3.11%,P〈0.05）. HCG positive rate,clinical pregnancy rate and implantation rate in fresh D6 group had no significant differences with frozen-thawed D6 group（69.61% vs 68.33%,60.56% vs 59.87%,61.88% vs 60.95%,P〉0.05）;the early abortion rate in fresh D6 group was lower than that in frozen-thawed D6 group（5.50% vs 9.06%）,but there were no significant differences between the two groups（P〉0.05）. Conclusions Clinical pregnancy outcome of D6 single blastocyst transfer may not be as good as D5 blastocyst. Fresh transfer of D6 single blastocyst may have the similar or better clinical pregnancy outcome of its frozen-thawed transfer.
