摘  要:目的:探讨糖化血红蛋白(HbA1C)在糖代谢异常患者中的变化。方法:选取就诊的695例患者作为研究对象。按照WHO糖尿病委员会提出的诊断标准(1999)分组:正常组48例,糖耐量减低(IGT)组101例,空腹血糖受损(IFG)组8例,糖尿病(GM)组538例。结果:糖化血红蛋白在糖耐量减低组,空腹血糖受损组,糖尿病组较正常人群组高。受试者工作特征(ROC)曲线显示:HbA1C在GM组的切点为6.1%,敏感性为81.41,特异性为97.92,曲线下面积0.942,Youden指数0.793;HbA1C在IGT组的切点为5.8%,敏感性为62.38,特异性为83.33,曲线下面积0.774,Youden指数0.457;HbA1C在IFG组的切点为5.8%,敏感性为87.5,特异性为85.42,曲线下面积0.901,Youden指数0.729。结论:HbA1C可以作为糖尿病筛查的指标,糖尿病的诊断应以空腹血糖(FPG)及口服葡萄糖耐量(OGTT)Objective To discuss the glycosylated hemoglobin( HbA1C) changes in patients with abnormal glucose metabolism. Method 695 cases patients of our hospital that in order to make clear the diagnosis were selected as the research. According to the diagnostic criteria proposed by the WHO Diabetes Committee(1999),the patients were divided into four groups:Normal group( n = 48),IGT group( n = 101),IFG group( n = 8),GM group( n = 538). Results The HbA1 C in the IGT group,IFG group,and GM group was higher than the normal group. Receiver operating characteristic( ROC) curve display:The cutpoint of the HbA1 c in GM Group was 6. 1%,the sensitivity was 81. 41,the specificity was 97. 92,the areas under the curve was 0. 942 and the Youden index was 0. 793;The cutpoint of the HbA1 c in IGT group was 5. 8%,the sensitivity was 62. 38,the specificity of 83. 33,the areas under the curve was 0. 774 and the Youden index was 0. 457;The cutpoint of the HbA1 c in IFG cutpoint was 5. 8%,the sensitivity was 87. 5,the specificity was 85. 42,the areas under the curve was 0. 901 and the Youden index 0. 729. Conclusion The HbA1 C can be used as a diabetes screening index,and the diagnosis of diabetes should be combined with FPG,OGTT and HbA1 C.
