微型锚钉联合克氏针固定治疗手指伸肌腱止点撕脱伤的应用研究

摘　　要：目的探讨应用微型锚钉联合克氏针固定治疗手指伸肌腱止点撕脱伤的临床疗效。方法选择符合标准的56例手指伸肌腱止点撕脱伤患者,随机分为观察组与对照组各28例。观察组患者使用微型锚钉联合克氏针固定治疗,对照组患者使用钢丝抽出止点重建法治疗,比较两组患者的治疗效果、手术时间、平均住院日、恢复时间及住院费用。结果观察组的手术时间、平均住院日、恢复时间分别为(28.4±3.2)min、(5.4±2.1)d与(40.8±5.3)d,均显著少于对照组(41.4±4.2)min、(8.6±3.3)d与(51.2±5.8)d。观察组仅出现1例肌腱松弛。对照组出现2例指腹压疮、2例肌腱松弛及1例钢丝脱落。Dargan功能评定法结果表明,观察组的优良率为89.3%,显著高于对照组的57.2%。结论应用微型骨锚钉联合克氏针重建手指伸肌腱止点手术过程安全简单快速,术后优良率高,并发症少,患者功能恢复良好且顺应性好,值得临床广泛推广使用。Objective To investigate the clinical efficacy of micro-anchor repair combined with Kirschner wire fixation for extensor terminal tendon avulsion treatment. Methods A total of 56 patients with finger extensor terminal tendon avulsion were selected and randomized into observation group and control group, 28 cases in each group.The observation group received the treatment of micro-anchor repair combined with Kirschner wire fixation, while the control group received the treatment of reconstruction method of using wire pull out the button. The efficacy, operation time, the mean hospitalization time, recovery time and hospitalization expense were compared. Results The operation time, the mean hospitalization time and recovery time of the observation group were（28.4±3.2） min,（5.4±2.1） d and（40.8±5.3） d,which were significantly shorter than the control group numbered as（41.4±4.2） min,（8.6±3.3） d and（51.2±5.8） d. Only1 case of tendon relaxation occurred in observation group, while the control group had 2 cases of abdominal pressure ulcers, 2 cases of tendon relaxation and 1 case of steel wire falling off. The results of Dargan function evaluation showed that the excellent rate of the observation group was 89.3% and was significantly higher than the control group＇ 57.2%.Conclusion The application of micro-anchor repair combined with Kirschner wire fixation for finger extensor terminal tendon avulsion treatment is safe, reliable, simple and fast. The operation has higher excellent rate and less complications occurs. Patients have good functional recovery and good compliance. It is worthy of further optimization and promotion, and popularizing and applying in clinical.
