糖尿病患者外周动脉病变与踝肱指数的相关性及危险因素分析

摘　　要：目的分析踝肱指数(ABI)与2型糖尿病患者发生外周动脉病变(PAD)的相关性,对外周动脉病变发生的危险因素进行分析,探讨ABI作为评价外周动脉病变简易指标的可靠性。方法对2015年1月至2015年4月合肥市第三人民医院内分泌科收治的200例2型糖尿病患者进行ABI测定,以0.9≤ABI<1.3为正常组,ABI<0.9为低ABI组,对两组患者糖尿病病程、年龄、血压、血糖、血脂、血肌酐、尿白蛋白/肌酐等指标进行比较,分析ABI值与上述指标的相关性。结果低ABI组年龄、病程、餐后2 h血糖、糖化血红蛋白、三酰甘油、收缩压、尿白蛋白/肌酐、高敏C反应蛋白均高于正常ABI组(P<0.05)。年龄(OR=1.21,95%CI:1.08~1.13)、低密度脂蛋白胆固醇(OR=1.43,95%CI:1.27~1.53)、尿白蛋白/肌酐(OR=1.29,95%CI:1.01~2.33)是ABI降低的危险因素。结论 ABI降低与多种导致动脉粥样硬化的因素有关,ABI测定可作为预测糖尿病PAD的简易可靠指标。Objective To analyse the correlation between ankle-brachial index （ABI） and peripheral arterial disease（PAD）in patients with type 2 diabetes and discuss the risk factors of PAD and analyse the reliability of ABI as a simple evaluation index. Methods ABI was examined in the patients admitted to the Endocrinology Department of the Third People＇s Hospital of Hefei City between January and April in 2015. The patients with ABI 0.9~1.3 were set as normal ABI group and those with an ABI below 0.9 as the low ABI group. Variables between these two groupswere compared, such as the course of diabetes, age, blood pressure, blood glucose, blood fat, serum creatinine and urinary albumin/creatinine, and the relativity of ABI to these factors was analyzed. Results The age, course of diabetes, 2-hour postprandial blood glucose, glycosylated hemoglobin, triglyceride, systolic blood pressure, urinary albumin/creatinine, hypersensitive C-reactive protein of low ABI group were higher than those of normal ABI group （P〈0.05）.Age（OR=1.21, 95%CI：1.08-1.13）,low density lipoprotein cholesterin （OR=1.43, 95%CI：1.27-1.53）,urinary albumin/creatinine（OR=1.29, 95%CI：1.01-2.33）were the risk factors of low ABI. Conclusion Low ABI is associated with multiple factors that lead to atherosclerosis, thus it could be used as a simple and reliable index for diabetes PAD prediction.
