中频脉冲电刺激治疗联合常规康复训练治疗脑卒中后肩关节半脱位的疗效观察

摘　　要：目的探讨中频脉冲电刺激治疗联合常规康复训练治疗脑卒中后肩关节半脱位的临床疗效。方法 40例脑卒中后肩关节半脱位患者作为研究对象,按入组顺序分为研究组和对照组,每组20例。所有患者均给予血糖、血压控制等常规内科治疗,对照组在常规治疗的基础上接受常规康复训练,研究组在对照组的基础上加用中频脉冲电刺激治疗。统计评价两组的临床疗效、患侧肩关节恢复情况及上肢运功功能改善情况。结果治疗结束后,对照组总有效率70.0%低于研究组95.0%,差异具有统计学意义(P<0.05)。治疗后,对照组的Fugl-Meyer评分和Barthel指数评分分别为(14.12±1.46)分和(10.37±1.21)分,研究组分别为(19.86±1.62)分和(18.27±1.06)分,研究组Fugl-Meyer评分和Barthel指数评分均高于对照组,差异均有统计学意义(P<0.05)。结论中频脉冲电刺激治疗联合常规康复训练治疗脑卒中后肩关节半脱位的临床疗效显著,值得临床推广应用。Objective To explore clinical effect of medium frequency pulse electrical stimulation combined with conventional rehabilitation training in treating shoulder joint half dislocation caused by stroke. Methods A total of 40 patients with shoulder joint half dislocation caused by stroke were divided by admission order into research group and control group, with 20 cases in each group. All patients received conventional medical treatment such as blood glucose and blood pressure controlling. The control group also received conventional rehabilitation training, and research group received medium frequency pulse electrical stimulation compound conventional rehabilitation training. Comparison were made on clinical effect, affected side shoulder joint recovery and upper limb exercise function improvement. Results The control group had a lower total effective rate as 70.0% after treatment than 95.0% in research group,and difference had statistical significance（P〈0.05）. The control group had Fugl-Meyer score and Barthel index score respectively as（14.12±1.46） points and（10.37±1.21）points after treatment, which were respectively（19.86±1.62） points and（18.27±1.06）points in research group.The research group had higher Fugl-Meyer score and Barthel index score than control group,and difference had statistical significance（P〈0.05）. Conclusion Medium frequency pulse electrical stimulation combined with conventional rehabilitation training in treating shoulder joint half dislocation caused by stroke shows significant clinical effect and this method is worth clinical promotion and application.
