摘 要:目的:探讨针刺联合丰富康复训练对脑缺血大鼠神经功能的影响及对神经再生抑制因子勿动蛋白(Nogo-A)、勿动蛋白受体(Ng R)的调控作用。方法:健康雄性SD大鼠120只,按照随机数字表分为造模组96只和假手术组24只,造模组采用颈外动脉线栓法制作局灶性大脑中动脉阻塞大鼠模型,术后4 h选择神经功能缺损评分为1~3分的大鼠96只,随机分为模型组、针刺组、丰富康复训练组、针刺+丰富康复训练组,每组24只。比较术后4 h、3 d、7 d、14 d 5组神经缺损体征评分和感觉、运动功能评分,采用免疫组化法检测Nogo-A蛋白及Ng R在大鼠脑梗死区海马中的表达。结果:术后7 d、14 d,神经缺损体征评分和感觉、运动功能评分比较,针刺组、丰富康复训练组、针刺+丰富康复训练组均低于模型组(P<0.01或P<0.05);其中针刺+丰富康复训练组低于针刺组、丰富康复训练组(P<0.01或P<0.05)。术后3 d、7 d、14 d,Nogo-A、Ng R蛋白表达比较,模型组、针刺组、丰富康复训练组、针刺+丰富康复训练组均高于假手术组(P<0.01或P<0.05),针刺组、丰富康复训练组、针刺+丰富康复训练组低于模型组(P<0.01或P<0.05),针刺+丰富康复训练组低于针刺组、丰富康复训练组(P<0.01或P<0.05)。结论:针刺联合丰富康复训练可改善脑缺血大鼠海马病理性损害,促进感觉、运动功能恢复,且针刺联合丰富康复训练效果优于单纯针刺治疗和单纯丰富康复训练。Objective: To explore the effect of acupuncture combined with enriched rehabilitation training on the neural function, the study would observe the regulation of nerve regeneration inhibiting factor (Nogo-A) and Nogo receptor (NgR) in cerebral ischemia rats. Methods :Healthy male SD rats were randomly divided into operation group and sham operation group. Cerebral ischemia model of middle cerebral artery occlusion (MCAO) was established in rats by using the external carotid artery embolism. Four hours a^er the surgery, 96 rats of one to three points of NIHSS were selected and divided into the model group, the acupuncture group, the enriched rehabilitation training group and the acupuncture combined with enriched rehabilitation training group, and 24 rats in each group. The scores of neural function deficit, sensation and motor ability were recorded, and the expression of Nogo-A and NgR proteins in hip- pocampal tissues were detected by immunohistochemistry staining. Results : At the 7 d, 14 d after treatment, the neural function deficit score, sensation and motor ability score in the acupuncture group ,the enriched rehabilitation training group and the acupuncture com- bined with enriched rehabilitation training group were lower respectively than those of the model group (P〈 0.01 or P〈 0.05 ), and the scores in the acupuncture combined with enriched rehabilitation training group were lower respectively than those of the acupuncture group and the enriched rehabilitation training group (P〈0.01 or P〈0.05 ). The expression of Nogo-A and NgR proteins in the model group, the acupuncture group ,the enriched rehabilitation training group and the acupuncture combined with enriched rehabilitation training group were higher respectively than those of the sham operation group (P〈0.01 or P〈0.05 ) ,the proteins expression in the acupuncture group, the enriched rehabilitation training group and the acupuncture combined with enriched rehabilitation training group were lower respectively t
