不同剂量甲状腺素治疗碘131治疗后甲状腺功能减退症的效果对比

摘　　要：目的探析不同剂量甲状腺素治疗碘131治疗后甲状腺功能减退症的效果。方法选择2015年7月至2016年6月在本院接诊并进行治疗的甲减患者94例,患者均经碘131治疗,并且治疗后出现甲状腺功能减退的症状,将患者随机分成研究组和对照组,各47例,研究组给予小剂量甲状腺素治疗,对照组给予大剂量的甲状腺素治疗,治疗4周后观察患者血清中游离的三碘甲状腺原氨酸(FT3)水平、游离的甲状腺素(FT4)水平以及促甲状腺激素(TSH)水平。结果治疗后,研究组的总有效率(93.62%)明显高于对照组(65.96%),两组甲减患者治疗效果差异显著,具有统计学意义(P<0.05);研究组和对照组血清中的FT3、FT4水平明显高于治疗前,治疗前后差异显著,具有统计学意义(P<0.05),两组患者治疗后的TSH水平明显低于治疗前,差异具有统计学意义(P<0.05),研究组和对照组的FT3水平、FT4水平以及TSH水平与对照组差异显著,具有统计学意义(P<0.05)。结论小剂量甲状腺素治疗碘131治疗后甲状腺功能减退症的效果显著,使得患者甲状腺功能恢复正常,临床上可推广小剂量的甲状腺素进行治疗。Objective To explore the effect of different doses of thyroidhormone in the treatment ofhypothyroidism after the treatment of iodine 131.Methods 94 patients withhypothyroidism after treated by iodine 131 from July 2015 to June 016 in ourhospital,were randomly divided into study group and control group,47 cases in each group,giving small doses of thyroidhormone therapy,the control of thyroxine treatment the observation group were given large doses,three free triiodothyronine in serum of patients after 4 weeks of treatment（FT3）,free thyroxine（FT4）and the level of thyroid stimulatinghormone（TSH）level.Results After treatment,the total efficiency of the study group（93.62%）was significantlyhigher than the control group（65.96%）,the differences between the two groups of patients withhypothyroidism treatment significantly,with statistical significance（P〈0.05）; the study group and the control group in serum FT3 and FT4 levels were significantlyhigher than that before treatment, the difference before and after treatment was statistically significant.The significance（P〈0.05）,the two groups after treatment TSH levels were significantly lower than that before treatment,the difference was statistically significant（P〈0.05）,the study group and the control group FT3 level,FT4 level and TSH level significantly compared with the control group,with statistical significance（P〈0.05）.Conclusions The effect of small doses of thyroidhormone inhypothyroidism patients after treated by iodine 131 is significant,the patient＇s thyroid function returned to normal; Clinical application of small doses of thyroidhormone inhypothyroidism patients after treated by iodine 131 can be popularized.
