不同插管方法在缺齿松动牙齿老年患者全麻中的应用

摘　　要：目的比较普通直视喉镜、光棒和纤维支气管镜在缺齿松动牙齿老年患者气管插管中的应用效果。方法选择90例缺齿松动牙齿老年患者,随机均分为三组:普通喉镜组(A组)30例、光棒组(B组)30例和纤支镜组(C组)30例,记录并比较三组患者气管插管成功率、平均气管插管操作时间和气管插管后并发症。结果三组患者气管插管成功率比较,差异无统计学意义(P>0.05),B组气管插管操作时间短于A组和C组,差异有统计学意义(P<0.05),三组气管插管并发症(牙齿牙龈损伤、术后咽喉疼痛和声嘶情况)比较,差异无统计学意义(P>0.05)。结论光棒气管插管在缺齿松动牙齿老年患者中气管插管操作时间短,麻醉气管插管并发症低,操作简便,迅速有效。Objective To compare the application effect of ordinary direct vision laryngoscope ,light stick and fiberoptic bronchoscope on tracheal intubation in aged patients with missing and loose teeth. Methods A total of 90 patients with missing and loose teeth were selected and randomly divided into three groups： 30 cases in the ordinary direct vision laryngoscope group （group A）, 30 cases in the light stick group （group B） and 30 cases in fiberoptic bronchoscope group （group C）. The success rate of the first attempt at endotracheal intubation, average intubation time and postoperative intubation complications were recorded and compared among the three groups. Results There was no significant differ- ence in the success rate of intubation among three groups （P〉0.05）. The time of intubation in group B was shorter than that in group A and C （P〈0.05）, and the difference was statistically significant （P〈0.05）. The intubation complications including gums, teeth, postoperative sore throat and hoarseness of the three groups, had no significant difference（P〉0.05）. Conclusion Light bar tracheal intubation time is short in missing and loose teeth of elderly patients. The complications of anesthesia tracheal intubation is low, and the operation of anesthesia intubation is simple, rapid and effective.
