2组不同剂量丙种球蛋白治疗重症手足口病的疗效研究

摘　　要：目的观察2组不同剂量丙种球蛋白对重症手足口病(hand-foot-mouth disease,HFMD)的疗效研究。方法选取487例手足口病患者,随机分为静脉丙种球蛋白足量组313例和静脉丙种球蛋白半量组174例,2组均依据《手足口病诊疗指南2013年版》重症治疗原则给予治疗。观察患者体温消退时间、其他症状、不良反应、平均住院时间和平均住院费用等情况。结果 2组在退热和皮疹消退方面,差异均无统计学意义(P>0.05);2组平均住院时间的比较差异也无统计学意义;2组平均住院费用的比较,足量组的费用显著高于半量组的费用(P=0.008)。结论在临床治疗HFMD重症患者时,采用半量丙种球蛋白治疗既能缩减患者的费用,减轻经济负担,又能在住院时间及退热时间方面与丙种球蛋白足量治疗的疗效相当,值得临床推广。Objective foot-mouth disease（HFMD）. Methods In total of 487 cases of patients with HFMD were selected and were divided into full dose of intravenous gamma globulin group（n=313, the full dose group） and half dose of intravenous gamma globulin group（n=174, the half dose group）, and the two groups were given treatment in accordance with treatment principles of severe illness in the Diagnosis and Treatment Guidelines for Hand-foot-mouth Disease（201u3lt）s. The defervescence, other symptoms, adverse reaction, average length of stay and average hospitalization expenses were observed. ResThe treatment results indicated that the full dose group and the half dose group showed no statistically significant differences in therapeutic effects of defervescenceand deflorescence, as well as the average length of stay; while the difference in the average hospitalization expenses of the two groups showed statistical significance, with those in the full dose group being remarkably higher than those in the half dose group（P=0.008）. Conclusions In the case of treating patients with severe HFMD in clinic, half dose of gamma globulin can reduce the expenses of patients, alleviate economic burden, and have comparable therapeutic effects with the full dose of gamma globulin in terms of length of stay and defervescence, thus it is worthy of being promoted in clinic.
