互联网+四元联动延续护理整合照护模式的实践探索

摘　　要：目的：探讨互联网＋将社区、医院、居家护理、养老机构患者之间的护理行为进行整合分级的效果。方法：利用互联网构建四元联动信息框架，及时了解患者需求，给予快速响应。结果：透过四元联动延续护理整合照护模式，增加了患者与各机构之间的粘性，提高了患者的满意度，2015年1～5月和2016年1～5月比较，社区医联体单位上转患者增长34．10％，医院下转社区患者增长58．42％，居家护理新增85．00％。结论：四元联动延续护理整合照护模式延续护理实现了居家护理与院内护理和社区护理的无缝转接，不仅明确了各级医疗机构诊疗功能定位，而且便捷了医院、社区、养老机构、家庭中需要医疗服务的人群，使患者享受全人全程的照护。Objective：To discuss the effect of integration and classification of nursing behaviors of patients in Interact plus ＂four - aspect coordination＂ （ community, hospital, home, and nursing institution for the aged） continued nursing integration model. Methods ： A ＂four - aspect coordination＂ integrated information framework was built up via Internet, to timely understand the patient demands and give rapid responses. Results：Through ＂four- aspect coordi- nation＂ continued nursing integration model, the viscidity between patients and each institution was increased and the patent satisfaction was improved. Through comparison between January to May 2015 and January to May 2016, it was discovered that the patients transferred from community health centers to hospitals increased by 34.10%, the patients transferred from hospitals to community health centers increased by 58.42%, and the patients nursed at home increased by 85.00%. Conclusion：Through ＂four - aspect coordination＂ continued nursing integration model, the seamless connection between home care, hospital care, and community care was realized, and also it specified the location of diagnosis and treatment functions of medical institutions at all lev- els and provided convenience to the people needing medical service in hospitals, communities, nursing institutions for the aged, and homes, to make the patients enjoy the watched care in the whole course.
