大剂量丙种球蛋白治疗中、重度病毒性心肌炎的效果分析

摘　　要：目的探讨大剂量丙种球蛋白治疗中、重度病毒性心肌炎的效果。方法选取我院中、重度病毒性心肌炎60例患者,随机分为两组,分别为观察组(30例)和对照组(30例),对照组患者给予常规治疗,观察组在此基础上使用大剂量的丙种球蛋白治疗。比较两组患者治疗前后的心肌酶谱水平,指标包括CK(肌酸激酶)、LDH(乳酸脱氢酶)、CK-MB(肌酸激酶同工酶)水平;观察比较两组患者治疗前后的心电图改善情况、不良反应以及两组的临床疗效。结果观察组治疗后的心肌酶谱各项指标与对照组比较明显改善较好(P<0.05);观察组治疗后心电图改善率为90%与对照组的70%明显较高(P<0.05);观察组患者治疗后出现不良反应的有2例(6.7%),对照组有5例(16.7%),两组对比差异明显(P<0.05)。观察组的治疗总有效率为93.3%,明显高于对照组的73.3%(P<0.05)。结论大剂量丙种球蛋白在治疗中、重度病毒性心肌炎中具有较好的临床效果,能有效改善患者的临床症状,且使用安全性较高,因此值得临床的广泛应用。Objective To explore the effect of large dose of gamma globulin in treatment of moderate and severe viral myoearditis. Methods 60 cases of patients with moderate and severe viral myocarditis were selected and randomly divided into observation group （30 cases） and control group （30 cases）. Patients in control group were treated with conventional therapy, and patients in observation group were treated with large dose of gamma globulin on the basis of control group group. Muscle enzyme levels, iincluding CK （creatine kinase）, LDH （lactate dehydrogenase）, CK-MB （creatine kinase isoenzyme） levels of the two groups before and after the therapy were compared. ECG improvement, adverse reactions and clinical efficacy of the two groups before and after the therapy were observed and compared. Results Compared with control group, various indexes of myocardial enzyme spectrum after treatment of observation group were significantly improved （P 〈 0.05）. ECG improvement rate after the therapy of observation group was 90%, which was significantly higher than that of control group with 70% （P 〈 O.05）.Tbere were two cases appearing adverse reactions in observation group （6.7%）, and there were five cases in control group （16.7%）. The difference was significant （P 〈 0.05）. The total effective rate was 93.3% in the observation group, which was significantly higher than that in control group with 73.3% （P 〈 0.05）. Conclusion The effect of large dose of gamma globulin in treatment of moderate and severe viral myocarditis is good. It can effectively improve the clinical symptoms of patients and has high using security. So it is worthy of clinical application.
