超薄股前外侧穿支皮瓣与传统皮瓣的比较研究

摘　　要：目的：应用超薄股前外侧穿支皮瓣游离移植修复软组织缺损的临床体会。方法：回顾性分析自2014年7月～2015年12月开始开展超薄股前外侧穿支皮瓣游离移植修复手足软组织缺损。目前共行股前外侧穿支皮瓣手术12例，其中男8例、女4例，手部软组织缺损5例，足踝部软组织缺损7例。术后密切观察皮瓣血运，肤色，肿胀及外观状况。11例患者皮瓣全部成活，且外形满意，其中2例感觉神经吻合，股前外侧皮神经与胫神经感觉支吻合。结果：术后皮瓣除1例失败外全部成活，随访3～18个月，平均12个月，皮瓣质地柔软色泽好，耐磨，无臃肿，外观良好。结论：应用超薄股前外侧穿支皮瓣游离移植修复手足软组织缺损，皮瓣具有血管解剖位置恒定，创伤小，质地柔软色泽好，不臃肿，不影响穿鞋，外观良好，治疗效果满意。Objective：To application of thin strands of free transplantation of anterolateral perforators flap to repair the brotherhood of soft tissue defect of experience .Methods：Since 2014 were retrospectively analyzed from July to December 2015 , began to carry out the thin strands of free transplantation of anterolateral perforators flap to repair soft tissue defect in the hands and feet .Line at present , a total of 12 cases of femoral anterolateral perforators flap surgery , the male 8 cases, female 4 cases, hand soft tissue defect in 5 cases, 7 cases of foot soft tissue defect .Postoperative close observation flap blood supply , swelling of the skin tone ,and appearance status .All trans-planted tisssues survived , 11 patients and satisfactory appearance , Two cases of sensory nerve anastomosis , the shares before the sensory branch of the lateral cutaneous nerve and tibial nerve anastomosis .Rseults：Postoperative flaps all survived except one case of failure , and followed up for 3 ~18 months, an average of 12 months, the flap was soft good color , wear-resisting, no fat, look good.Conclusion：Application of thin strands of free transplantation of anterolateral perforators flap to repair extremities soft -tissue defects , the flap had a constant vascular anatomy position , small trauma, texture was soft, good color, not bloated, not affect wearing shoes, look good, the treatment effect was satisfactory .
