评估式管理方法在开胸术后胸腔闭式引流护理中的作用

摘　　要：目的探讨评估式管理方法在胸腔闭式引流护理工作中的效果。方法选取2012年1月-2013年1月在武装警察部队总医院行开胸手术并留置胸腔闭式引流管的98例患者，按照手术先后顺序随机分为对照组和观察组，对照组按照常规进行护理，观察组应用评估式管理的方法进行护理。结果观察组与对照组比较，常见并发症由28．0％下降至6．2％，差异有统计学意义（P〈0．05）。带管天数、住院时间观察组[（4．0±1．O）d、（7．5±1．4）d1与对照组[（4．6±1．2）a、（8．2±1．9）d]差异亦有统计学意义（均P〈0．05），住院费用观察组显著低于对照组[（6．6±1．4）万元比（7．2±1．0）万元，（P〈0．01），差异有统计学意义。结论评估式管理应用于临床能够降低开胸术后患者的并发症，同时降低了带管时间、住院时间和住院费用，提高了护理质量。Objective To investigate the effects of of evaluation management in nursing of thoracic close drainage after thoracotomy. Methods 98 patients undergoing thoracic close drainage after thoracotomy were randomly appointed into 2 equal groups according to the sequence of operation： control group to be cared according to routine nursing procedures and observation group to be cared by evaluation management nursing, including interrogation and careful physical examinations, focusing on the drain and drainage tube, and strengthened communication between physicians and nurses. Results The morbidity of complications of the observation group was 6.2%, significantly lower than that of the contyrol group （28.0%, P 〈0.01）. The number of days with intubation and hospital stay time of the observatyion group were （3.96±1.05） d and （7.5±1.4） d respectivey, both significntly shorter than those of the control group [（4.6±1.2） d and （8.2±1.9） d respectively, both P 〈0.05]. The hospitalization expenses （66±14） thousand RMB, significantly less than that of the control group [（72± 10） thousand RMB, P〈0.01]. Conclusions Evaluation management in nursing of thoracic close drainage after thoracotomy caring effectively improves the quality of nursing and the outcomes of the patients.
