摘  要:目的研究分析高龄无移位股骨颈骨折采取人工股骨头置换术与内固定术治疗的临床效果。方法2013年10月至2015年5月收住本院的无移位股骨颈骨折患者56例为研究组,年龄均大于60岁,住院期间采取人工股骨头置换方案治疗。同期人院患者60例为对照组,均接受空心钉内固定治疗。随访12个月,采用Harries评分比较两组患者患侧髋关节功能及围手术期并发症发生情况。结果研究组术后6个月和12个月患髋优良率分别为92.9%(52/56)、83.9%(47/56),而对照组则分别为78.3%(47/60)、68.3%(41/60),研究组术后各时间点优良率均优于对照组(P〈0.05)。研究组患者围手术期并发症发生率显著低于对照组(5.4%vs.20.0%,P〈0.05)。结论对于高龄无移位股骨颈骨折,采取人工股骨头置换手术可有助于改善患髋功能,减少围手术期并发症。Objective To investigate the efficacy of artificial femoral head replacement and internal fixation in the treatment of non displaced femoral neck fracture in the elderly patients. Methods 56 elderly patients with non displaced femoral neck fracture (〉60 years old) in our hospital from October 2013 to May 2015 were selected as study group, all treated with artificial femoral head replacement. 60 patients treated with internal fixation were selected as control group. The complications and the hip function score were evaluated and analyzed. Results The excellent and good rate of hip joint in study group 6 and 12 months after operation were 92.9% (52/56) and 83.9% (47/56), and those in control group were 78.3% (47/60) and 68.3% (41/60), the excellent and good rate in study group was better than that in control group at each time point after operation (P〈0.05). The incidence of perioperative complications in study group was significantly lower than that in control group (5.4% vs.20.0%, P〈0.05). Conclusion Artificial femoral head replacement surgery can improve the hip function of elderly patients with non displaced femoral neck fracture, and reduce perioperative complications.
