摘 要:目的总结海南省人工耳蜗捐赠项目所有患者的初次筛查和术前筛查结果,分析术前筛查未通过的原因,探讨提高术前筛查通过率、提升医疗资源利用率的方法。方法回顾性分析2013年1月~2016年6月在海南省聋儿康复指导中心申请人工耳蜗植入患者为的术前筛查资料包括听力学检查(纯音测听、声导抗测试、畸变耳声发射(DPOAE)、听觉脑干诱发电位(ABR)、稳态听觉诱发电位(ASSR))、影像学检查(颞骨CT、颅脑MRI、内听道水成像)、精神智力评估、家庭经济与康复条件评估等。结果总计完成初次筛查467例,初次筛查通过293例,未通过者174例;初次筛查通过者进行术前筛查;术前筛查通过255例,未通过38例(耳蜗畸形6例,内听道狭窄3例,颞叶脑白质病3例,无言语语言基础的超龄语前聋15例,本人无手术意愿6例,无康复条件5例)。结论本组患者初次筛查通过率62.7%,术前筛查通过率为87%。人工耳蜗植入术前充分的筛查,对于明确耳蜗植入适应证、禁忌症、预防手术并发症及初步了解康复效果的具有非常重要的意义,同时也可提高捐赠人工耳蜗的利用率。Objective To summarize and analyze the results of primary screening and pre-operational screening of recepients of cochlear implantation donation in Hainan province. Analysis of the reasons for preoperative screening failed, to improve preoperative screening method pass rate,improve the utilization ratio of health resources. Methods The subjects are applicants who applied for Cochlear implantation under guidance of Hainan deafness rehabilitation center from Jan. 2013 to 3un. 2016. The screening programmes include auditory test (pure tone average, acoustic impedence, distortion product otoacoustic emission (DPOAE), auditory brain stem response (ABR), auditory steady state response (ASSR)), imagingstudies (temporal bone CT, cranial MRI, water imaging of internal acoustic canal), psychiatric and intellectual evaluation, and family economy and rehabilitation qualification evaluation. Results A total of 467 cases were screened, 293 of which passed primary screening, and 174 failed screening, which passed the primary screening for preoperative screening, a total of 293 cases completed, 255 of which passed screening, and 38 of which failed screening (6 cases of cochlear malformation, 3 cases of internal acoustic canal stenosis, 3 cases of temporal lobe white matter disease, 15 cases of over-aged pre-lingual deaf patients without language background, 6 cases without patients' consent, and 5 cases without qualification for rehabilitation). Conclusion The rate for passing primary screening was 62.7%,the rate for passing pre-operational screening was 87%. The pre-operational screening for cochlear implantation should be thorough, because it not only plays key role in clarifying indication and contra-indication for cochlear implantation, preventing surgical complication, and estimating rehabilitation outcome, but also can improve usage of donated cochlear implants.
