摘 要:目的探讨CDFI检测腹壁浅动静脉的应用价值,为腹壁浅动脉皮瓣的临床应用提供依据。方法采用CDFI随机检查60例健康志愿者双侧腹壁浅动静脉。观测两侧的出现率、走形层次、伴行关系、越过腹股沟韧带的位置距腹股沟韧带中点的距离及腹壁浅动脉的内径、PSV、PI。结果 120侧下腹壁检测到腹壁浅动脉102侧,显示率:85.00%,腹壁浅静脉120侧,显示率:100%。动静脉越过腹股沟韧带的位置距韧带中点分别为:(5.54±8.12)mm和(-6.75±9.39)mm。腹壁浅动脉起源内径(D1)为:(1.52±0.18)mm,越过腹股沟韧带内径(D2)为:(0.70±0.12)mm,PSV:(21.10±2.99)cm/s,PI:1.68±0.25。左右两侧动脉D1、D2、PSV、PI无统计学意义(P>0.05),男性的D1、D2、PSV、PI均高于女性(P<0.05)。结论 CDFI能清晰地显示及定位腹壁浅动静脉,具有操作简单、无损伤、重复性高等优点,可在术前对腹壁浅动脉皮瓣的血管进行评估,为术中皮瓣的设计提供影像学依据。Objective To explore the application value of color Doppler flow imaging(CDFI) investigating SIEA and SIEV and provide imaging evidence for preoperative flap design and clinical application. Methods Both sides of SIEA and SIEV in 60 normal volunteers were investigated with CDFI by the generation of random numbers. The occurrence rate, course, mean distances from the inguinal ligament midpoint of SIEA and SIEV, the diameter, peak systolic velocity (PSV) and Pulsated index (PI) of SIEA were recorded. Results One hundred and two sides of SIEA (85.00%) and 120 sides of SIEV (100.00%) were observed. The mean distances of SIEA and SIEV were (5.54±8.12) mm and (-6.75±9.39) mm from the midpoint of the inguinal ligament. The origin caliber of SIEA(D1) was (1.52±0.18)ram. The caliber of the inguinal ligament Level (D2) was (0.70±0.12)mm. PSV was (21.10±2.99) cm/s. PI was 1. 68±0.25. There were no significant differences in the diameter,flowing parameters between left and right superficial inferior epigastric arteries (P〉0.05), but these parameters, including D1, D2, PSV and PI were higher in the male than in the female (P〈0.05). Conclusions CDFI could show the locations of SIEA and SIEV clearly. As it has the advantages of simple operation,non-damage and good repeatability, it provides more scientific and accurate evidence for preoperative flap design and vascular assessment.
