摘　　要：目的研究肝脏上皮样血管平滑肌脂肪瘤(EAML)的超声表现特征。方法回顾性分析39例(41个病灶)经手术病理证实为肝EAML的声像图,分析其超声表现特征。结果肝EAML以中年女性多见。病灶大小1.5~16cm,平均(5.8±4.1)cm。超声表现为66%(27/41)病灶呈低回声,34%(14/41)呈高回声;44%(18/41)病灶内部回声不均匀;51%(21/41)边界清;17%(7/41)病灶周边可见低回声晕环。CDFI显示83%(34/41)病灶内有血流信号,其中46%(19/41)表现为周边和内部较丰富点线状血流(I型),血管扭曲杂乱,部分病灶周边有血管包绕;37%(15/41)病灶表现为周边和(或)内部少量点状、短线状血流(Ⅱ型),RI值为0.38~0.66,平均值0.50±0.10。5%(2/39)可出现局部复发和远处转移,为恶性肝EAML。结论肝EAML因其组织成分和血供特点,其灰阶声像图和CDFI表现具有一定特征性,可有助于诊断。Objective To analyze the ultrasound imaging features of epithelioid angiomyolipoma （EAML） of liver. Methods The ultrasound findings of 39 cases （41 lesions） with pathologically proven hepatic EAML were retrospectively analyzed. Results Hepatic EAML was more commonly found in middle-aged women. Tumor size ranged from 1. 5 to 16 cm （mean 5.8±4. 1 cm）. 66% （27/41） lesions were hypoechoic and 34% （14/41） lesions were hyperecho. 44% （18/41） lesions were inhomogeneous. 51% （21/41） lesions were well defined. 17 % （7/41） lesions had hypoechoic rim. CDFI showed that 83% （34/41） lesions were vascularity. 46% （19/41） lesions had rich color Doppler signal around and internally （I type） , which was thick and distorted. 37%（15/41） lesions had slight color Doppler signal （ II type）. RI value ranged from 0.38-0.66 （mean 0. 50±0.10）. 5% （2/39） lesions would be diagnosed as malignant hepatic EAML with recurrence and metastases. Conclusions Hepatic EAML has unique ultrasound features ascribed to the tissue and vascularity, which could be helpful in the diagnosis of hepatic EAML.
