新参数网织红细胞血红蛋白在珠蛋白生成障碍性贫血诊疗中的应用

摘　　要：目的研究网织红细胞血红蛋白在珠蛋白生成障碍性贫血诊疗中的应用。方法选择2014年7月至2015年12月该院接受检查的珠蛋白生成障碍性贫血患者2 400例、缺铁性贫血的患者500例,分别将其纳入珠蛋白生成障碍性贫血组和缺铁性贫血组,另纳入500例健康体检者为对照组同时对2组患者的红细胞参数和网织红细胞的6项参数进行比较。结果缺铁性贫血组和珠蛋白生成障碍性贫血组平均红细胞体积(MCV)、平均红细胞血红蛋白量(MCH)、平均红细胞血红细胞血红蛋白浓度(MCHC)明显低于对照组,红细胞体积分布宽度(RDW)明显高于对照组,差异有统计学意义(P<0.05);珠蛋白生成障碍性贫血组MCV、MCH、MCHC[(70.85±10.78)fL、(22.55±7.12)pg、(302.3±15.25)g/L]高于缺铁性贫血组[(68.32±10.84)fL、(20.54±4.68)pg、(30.15±8.82)g/L],RDW[(44.42±11.65)%]低于缺铁性贫血组[(54.65±7.92)%],差异有统计学意义(P<0.05);珠蛋白生成障碍性贫血组6项网织红细胞参数与缺铁性贫血组比较,差异均有统计学意义(P<0.05)。结论应用新参数网织红细胞血红蛋白能有效地提高珠蛋白生成障碍性贫血的筛查率,能够更好地为临床提供依据,建议将新参数网织红细胞血红蛋白作为珠蛋白生成障碍性贫血诊疗中的重要指标。Objective To explore the value of reticulocyte hemoglobin in the diagnosis and treatment of thalassemia.Methods A total of 2 400 patients with thalassemia and 500 patients with iron deficiency anemia from July 2014 to December 2015 were divided into thalassemia group and iron deficiency anemia group respectively,500 health persons were recruited into control group.red blood cell parameters and six parameters of reticulocyte were compared.Results Mean corpuscular volume（MCV）,average amount of red blood cell hemoglobin（MCH）,Red blood cell hemoglobin concentration（MCHC）in the iron deficiency anemia and thalassemia group were significant lower than those in the control group,red blood Cell distribution width（RDW）was significant higher than that of the control group,the difference was statistical significant（P〈0.05）.MCV,MCH,MCHC in the thalassemia group[（70.85±10.78）fL,（22.55±7.12）pg,（302.3±15.25）g/L]were higher than those in the iron deficiency anemia group[（68.32±10.84）fL,（20.54±4.68）pg,（30.15±8.82）g/L],RDW in the thalassemia group[（44.42±11.65）%]was lower than that in the iron deficiency anemia group[（54.65±7.92）%],the difference was statistical significant（P〈0.05）.Six parameters of reticulocyte in the thalassemia group were significant different with those in the iron deficiency anemia group（P〈0.05）.Conclusion Application of the new parameters of reticulocyte hemoglobin could effectively improve thalassemia screening rates,better able to provide the basis for clinical,it is recommend that new parameters reticulocyte hemoglobin used to be as thalassemia diagnosis and treatment of important indicators.
