摘  要:目的:比较毫针、电针、艾灸3种刺灸方法对功能性便秘大鼠的治疗作用差异,并探讨其疗效的可能机制。方法:雄性SD大鼠随机分为空白对照组(8只)、模型组(11只)、西沙必利组(8只)、毫针组(11只)、电针组(11只)和艾灸组(11只)。除空白对照组外,余各组每日予盐酸洛哌丁胺混悬液(3mg/kg)灌胃,制备功能性便秘大鼠模型。造模同时每日灌胃后间隔1h,西沙必利组予西沙必利混悬液灌胃,毫针组、电针组和艾灸组在"天枢"穴和"上巨虚"穴分别予以毫针、电针、艾灸干预治疗,每日1次,治疗15min,共治疗6d。观察各组大鼠首粒黑便排出时间,酶联免疫法检测血浆一氧化氮(NO)、一氧化氮合酶(NOS)、血管活性肠肽(VIP)的含量,免疫组化法、蛋白印迹法检测各组大鼠结肠组织中VIP的表达。结果:与空白对照组比较,模型组大鼠的首粒黑便排出时间延长(P<0.05),血浆NO、NOS、VIP含量均升高(P<0.01,P<0.05),VIP蛋白在大鼠结肠肠壁的平滑肌层表达评分升高(P<0.05),结肠组织VIP蛋白表达升高(P<0.01)。与模型组比较,西沙必利组、毫针组、电针组和艾灸组首粒黑便排出时间均缩短(P<0.05),血浆NO、NOS、VIP含量均降低(P<0.01,P<0.05),结肠组织VIP蛋白表达评分均降低(P<0.05),VIP蛋白表达均降低(P<0.01)。与西沙必利组比较,毫针组、电针组、艾灸组首粒黑便排出时间延长(P<0.05),电针组血浆VIP含量降低(P<0.01),艾灸组结肠组织VIP蛋白表达降低(P<0.01)。3个刺灸干预组之间比较,电针组血浆NO、NOS含量及结肠组织VIP蛋白表达评分均低于毫针组和艾灸组(P<0.01,P<0.05);血浆VIP含量由低到高依次为电针组、艾灸组、毫针组,且差异均有统计学意义(P<0.01,P<0.05);结肠组织VIP蛋白表达,艾灸组低于毫针组(P<0.05)和电针组(P<0.01)。结论:3种刺灸方法均对功能性便秘有良性调整作用,其中电针对血浆NO、NOS及VIP含量的调节作用较为显著,而在Objective To compare the effects of filiform needling,electroacupuncture,and moxibustion on functional constipation in rats,and to explore the possible mechanism of these three different methods.Methods Male SD rats were randomly divided into blank control group(n=8),model group(n=11),medication group(n=8),filiform needling group(n=11),electroacupuncture group(n=11),and moxibustion group(n=11).Functional constipation model was established by intragastric administration with the suspension of loperamide hydrochloride,daily for six days in a week.One hour after each gavage,the medication group was treated with cisapride suspension,while the other three groups were treated with filiform needling,electroacupuncture,and moxibustion,respectively,at"Tianshu"(ST 25),"Shangjuxu"(ST 37)acupoints.The first defecation time was recorded.NO,NOS and VIP(vasoactive intestinal peptide)levels in plasma were detected by ELISA.Immuohistochemical and Western blot methods were applied to test VIP expression in the colonic tissue.Results Compared with the blank control group,the first defecation time was prolonged,and the NO,NOS and VIP contents in plasma were increased in the model group(P0.05,P0.01),accompanied with increased score of VIP in the smooth muscle of colonic wall(P0.05)and elevated VIP expression in colonic tissue(P0.01).Compared with the model group,the first defecation time was shortened in the medication,filiform needling,electroacupuncture,and moxibustion groups(P0.05),while decreased NO,NOS and VIP contents in plasma(P0.01,P0.05),decreased score of VIP in the smooth muscle of colonic wall(P0.05),and lower expression level of VIP in colonic tissue(P0.01)were observed in the four treatment groups.In contrast,the plasma VIP content in the electroacupuncture group was lower than that in the medication group(P0.01),while colonic VIP expression decreased in the moxibustion group(P0.01).The NO,NOS and VIP contents in plasma and score of VIP in t
