稳定性冠心病患者对经皮冠状动脉介入术获益的预期现状及其影响因素的研究

摘　　要：目的了解稳定性冠心病患者对经皮冠状动脉介入术(percutaneous coronary intervention,PCI)可能获益的预期现状,分析其影响因素,为相关研究提供依据。方法采用方便抽样法,选取635例稳定性冠心病患者作为研究对象,采用一般资料调查表、欧洲五维健康量表、决策参与期待量表和医患关系量表对患者进行调查。结果仅21.57%的患者认为PCI不能治愈冠心病。女性、年龄较小、文化程度较低和医患关系较好的患者更倾向于相信PCI能够治愈冠心病,信息知识来源于网络的患者更倾向于相信PCI不能够治愈冠心病。结论多数稳定性冠心病患者对PCI存在错误的获益预期,预期与患者的性别、年龄、文化程度、医患关系及PCI信息知识来源有关。Objective To explore the patients＇ expectations about effects of percutaneous coronary intervention （PCI） for stable coronary artery disease and its influencing factors,and to provide evidence for related studies. Methods Convenience sampling method was used and a total of 635 patients diagnosed with stable coronary artery disease were investigated using general information questionnaire,European Quality of Life-5 Dimensions,Control Preference Scale and The Patient-Doctor Relationship Scale. SPSS 17.0 statistical software was used to analyze data. Results Only 21.6% of the patients correctly reported that PCI was not able to cure coronary artery disease. Female （ OR =3.102,95 % CI 1.808 -5.322）, younger age （OR =0.484,95 % CI 0.343 -0.684）, low educational level （ OR = 0.486,95%CI 0.370-0.637 ） and better patient-doctor relationship （OR = 1.576,95%CI 1.198-2.073 ） were significantly associated with inaccurate understandings,while patients were less likely to provide inaccurate responses if they received information of PCI from the Internet（OR=0.390,95%CI 0.212-0.717）. Conclusion A large proportion of patients with stable coronary artery disease have unrealistic expectations about effects of PCI. Gender,age,educational level,patient-doctor relationship and information source are independent predictors for expectations.
