复方利多卡因乳膏超前镇痛在低压无针注射器瘢痕内注射中的应用

摘　　要：目的探讨复方利多卡因乳膏超前镇痛应用于低压无针注射器增生性瘢痕内药物注射过程中患者的疼痛、应激反应及不良反应。方法选取增生性瘢痕患者90例,随机分为实验组和对照组,每组45例。实验组在与对照组相同治疗方案实施前1 h,于注射瘢痕区域外涂复方利多卡因乳膏。对照组采用低压无针注射器向瘢痕内注射盐酸利多卡因注射液和复方倍他米松注射液进行治疗。观察并分析两组治疗前、治疗中及治疗后10 min患者疼痛视觉模拟评分、平均动脉压、心率变化及不良反应。结果两组治疗前疼痛视觉模拟评分、血压、心率的差异无统计学意义(P>0.05);治疗中对照组疼痛视觉模拟评分、血压、心率均较实验组明显升高(P<0.05);两组治疗前后均未观察到明显的不良反应。结论无针注射器瘢痕内注射前给予复方利多卡因乳膏可产生良好的超前镇痛效果,降低注射过程中的神经系统敏感性,增强治疗过程中的舒适度,安全性较好。Objective To investigate the pain degree,stress response and adverse reactions of preemptive analgesia of lidocaine-prilocaine cream on hypertrophic scars injection with low pressure needle free syringe. Methods Ninety cases with hypertrophic scars were randomized into the treatment group and the control group,with 45 cases in each group. Patients in the control group were treated with Lidocaine Hydrochloride and Compound Betamethasone by injection with low pressure needle free syringe through scars. Patients in the treatment group received external application of lidocaine-prilocaine cream 1 hour prior to the same injection in the control group. Both groups were treated with nursing intervention combined with routine nursing. Pain visual analogue scale（VAS）,mean arterial pressure（MBP） ,heart rate（HR） and adverse reactions were observed before treatment,within treatment and 10 minutes after treatment. Results There was no significant difference in VAS,MBP and HR between two groups before treatment. VAS,MBP and HR were significantly higher in the control group than those in the treatment group（P〈0.05）. No specific adverse reaction was observed in two groups before and after treatment. Conclusion It works very well for preemptive analgesia of lidocaine-prilocaine cream on hypertrophic scars injection with low pressure needle free syringe. External application of lidocaine-priloeaine cream 1 hour prior to the hypertrophic scars injection could lower the sensitivity of sensory nerve endings,enhance the comfort of patient and provide better safety.
