益气化瘀方对髋关节置换术后血液高凝状态影响的临床研究

摘　　要：目的观察益气化瘀方对髋关节置换术后患者血液高凝状态的干预作用。方法将60例髋关节置换术后患者随机分为治疗组与对照组,每组30例。对照组予低分子肝素治疗,治疗组在对照组治疗措施基础上加予益气化瘀方治疗。两组疗程均为7天,观察比较D-二聚体(D-D)、纤维蛋白降解产物(FDP)、凝血酶原时间(PT)、部分凝血活酶时间(APPT)的变化情况。结果 1组内术后第1天与第7天比较,D-D、FDP水平均显著下降,差异有统计学意义(P<0.05)。组内术前与术后第7天比较,D-D、FDP水平差异无统计学意义(P>0.05)。组间术后第7天比较,治疗组D-D、FDP水平较对照组下降更显著(P<0.05)。2组内术前、术后第1天与第7天比较,PT、APTT差异无统计学意义(P>0.05)。组间术后第7天比较,PT、APTT差异无统计学意义(P>0.05)。结论益气化瘀方可明显改善髋关节置换术后患者血液高凝状态,预防深静脉血栓,且副作用较小。Objective To observe the intervention effects of Yiqi Huayu Recipe on the hypercoagulability in patients after hip arthroplasty. Methods 60 patients after hip arthroplasty were randomly divided into the treatment group and control group,30 cases in each group. The control group was treated with low molecular weight heparin,and the treatment group was treated with Yiqi Huayu Recipe based on the treatment for the control group,with a course of 7 days. The D-dimer（ D-D）,fibrin degradation product（ FDP）,prothrombin time（ PT） and partial thromboplastin time（ APTT） were observed and compared. Results 1The levels of D-D and FDP on 7th day after operation were both significantly decreased in the two groups compared with 1st day after operation,with statistically significant differences（ P〈 0. 05）. For the comparison between operation before and 7th day after operation within group,the differences on the levels of D-D and FDP were not statistically significant（ P 〉0. 05）. On 7th day after operation,the decrease of D-D and FDP levels in the treatment were more significant than those in the control group（ P 〈0. 05）. 2For the comparison between operation before and 1st and 7th day after operation within group,the differences on the PT and APTT were not statistically significant（ P 〉0. 05）. For the comparison between groups on 7th day after operation,the differences on the PT and APTT were not statistically significant（ P 〉0. 05）. Conclusion Yiqi Huayu Recipe can significantly improve the hypercoagulability in patients after hip arthroplasty,and prevent deep vein thrombosis with slight side effect.
