围手术期限制性输血的节约用血效果及成本效益分析

摘　　要：目的分析围手术期限制性输血的节约用血效果及成本效益。方法抽取粤北人民医院2015-04—2016-04间围手术期86例患者进行本次研究,将患者按数字表法分为对照组和观察组,每组43例患者。对照组患者围手术期给予开放性输血;观察组患者围手术期则针对用血节约性及成本效益给予限制性输血。对比两组患者的节约用血效果及成本效益。结果观察组患者的输血量(2.11±1.19)U及不良反应发生率明显低于对照组患者的输血量(3.13±1.45)U及不良反应发生率(9.30%),观察组患者的住院天数(20.09±6.24)d及输血成本(7124.56±365.13)元明显少于对照组患者的住院天数(28.57±6.43)d及输血成本(8743.25±375.23)元,差异有统计学意义(P<0.05)。结论围手术期患者限制性输血有效节约血资源的同时不良反应少,明显减少了住院天数,收到良好的效果,有效降低了输血成本,具有重要的意义。Objective To analyze blood conservation effectiveness and cost-effectiveness of perioperative restrictive transfusion. Methods 86 patients during perioperative period selected from the hospital were included in the study and assigned into control group and observation group, with 43 patients in each group. Patients in the control group were given liberal transfusion, and patients in the observation group were given restrictive transfusion for blood conservation and cost-effectiveness. Blood conservation effect and cost-effectiveness in the two groups of patients were compared. Results The volume of blood transfusion （2.11 ± 1.19） U and the incidence of adverse reactions in the observation group were significantly lower than that in the control group （3.13 ±1.45） U and （9.30%）. The hospitalization stay （20.09± 6.24） days and blood transfusion costs（7124.56±365.13） yuan in the observation group were significantly less than those in the control group （28.57 ± 6.43） days and （8743.25 ±375.23） yuan. The difference was statistically significant （P 〈 0.05）. Conclusion Restrictive blood transfusion can effectively conserve blood resources, induce few adverse reactions, significantly reduce the hospitalization stay, achieve good outcome, and effectively reduce blood transfusion cost in perioperative patients, which contribute great significance.
