竹圈盐灸治疗颞下颌关节炎临床随机对照研究  

Bamboo-circled Salt-partitioned Moxibustion for Arthritis of Temporomandibular Joint: A Randomized Controlled Clinical Trial

摘　　要：目的观察竹圈盐灸治疗颞下颌关节炎的临床疗效。方法将80例患者随机分为两组。竹圈盐灸组40例,将竹圈盐灸置于颞下颌关节处熨灸;温针灸组40例,选取患侧下关、阿是穴等穴,行温针灸。两组患者均以3 d为1个疗程,治疗2个疗程后统计疗效,观察治疗前后在疼痛及开口度方面的改善情况及1个月后的随访疗效。结果竹圈盐灸组在即刻镇痛方面明显优于温针灸组(P<0.05);在改善开口度方面亦优于温针灸组(P<0.05);竹圈盐灸组与温针灸组综合疗效显效率分别为67.5%和45.0%,差异有统计学意义(P<0.05),表明竹圈盐灸组在治疗颞下颌关节炎方面优于温针灸组;而两组治疗后随访结果均有良好的临床疗效,竹圈盐灸组有效率为92.5%,温针灸组有效率为87.5%,两组比较差异无统计学意义(P>0.05)。结论竹圈盐灸治疗颞下颌关节炎,有即刻镇痛和改善开口度的效果,具有疗效好、安全性高、无创伤、简便易行等优点,对惧针的患者尤为适宜。Objective To observe the clinical efficacy of bamboo-circled salt-partitioned moxibustion in treating arthritis of temporomandibular joint. Method Eighty patients were randomized into two groups. Forty cases in the bamboo-circled salt-partitioned moxibustion group received bamboo-circled salt-partitioned moxibustion at temporomandibular joint; forty cases in the warm needling group were intervened by selecting Xiaguan （ST7）, Ashi point, etc. at the affected side. For the two groups, 3-day treatment was taken as a treatment course, and the therapeutic efficacy was analyzed after 2 treatment courses. The improvements in pain and mouth opening were observed before and after the treatment, and the treatment efficacy was evaluated by a 1-month follow-up study. Result The bamboo-circled salt-partitioned moxibustion group was superior to the warm needling group in comparing the real-time analgesic effect （P〈0.05） and in the improvement of mouth opening （P〈0.05）; the comprehensive markedly effective rate was respectively 67.5% and 45.0% in the bamboo-circled salt-partitioned moxibustion group and warm needling group, and the between-group difference was statistically significant （P〈0.05）, indicating that bamboo-circled salt-partitioned moxibustion is better than warm needling in treating arthritis of temporomandibular joint; the follow-up study revealed satisfactory therapeutic efficacies in both groups： the effective rate was 92.5% in the bamboo-circled salt-partitioned moxibustion group versus 87.5% in the warm needling group, and the difference was statistically insignificant （P〉0.05）. Conclusion Bamboo-circled salt-partitioned moxibustion can produce a real-time analgesic effect and improve mouth opening; it＇s especially suitable to treat the patients who are afraid of needling, as it＇s significantly effective, safe, non-invasive, and easy-to-operate.
