人乳头瘤病毒型别与儿童复发性呼吸道乳头状瘤临床病程的相关性

摘　　要：目的研究人乳头瘤状病毒(human papilloma vir us,HPV)型别对儿童复发性呼吸道乳头瘤(juvenile onset recurrent respiratory papillomatosis,JORRP)临床病程的影响。方法收集38例JORRP患儿新鲜瘤体标本,采用流式荧光杂交法检测HPV型别。将患儿分为HPV6阳性组和HPV11阳性组。量化评分其201例手术,对发病年龄、并发症、临床症状、手术次数、乳头瘤侵袭范围程度等方面进行统计学分析。结果 55.2%(21/38)JORRP患儿感染HPV6,36.8%(14/38)感染HPV11,7.9%(3/38)HPV6/11均阴性。两组患儿在发病年龄、术前临床症状评分、乳头瘤解剖亚区数、Derkay、Dikkers评分方面均有统计学差异。结论感染HPV11发病年龄更小,侵犯范围更广病灶性状更严重,临床症状更严重,致JORRP临床病程更严重。OBJECTIVE To study on the influence of human papilloma virus（HPV） type on the clinical course of juvenile onset recurrent respiratory papillomatosis（JORRP）.METHODS Fresh tumor specimens of 38 patients were collected and the HPV type of the specimens was detected by flow fluorescent hybridization method.According to the results,children were divided into HPV6 and HPVl1 positive groups.The clinical data of the two groups were compared.RESULTS Among 38 children with recurrent respiratory papillomatosis,21（55.2%）were infected with HPV6,14（36.8%） were infected with HPV11,and 3（7.9%） were negative for HPV 6 or HPV11.The proportion of aggressive cases in HPV6 and HPV11 groups were similar.The age of onset,preoperative clinical symptom score,number of anatomic locations,anatomic Derkay/Coltrera score and Dikkers score were significant different between the two groups（P=0.002,0.040,0.023,0.001,0.005,respectively）.CONCLUSION JORRP patients with HPV11 infection had the features of smaller onset age,more severe clinical symptoms and broader invasion compared with HPV6 infection.
