摘  要:目的对辽宁省丹东市2015年8月发生的2例间日疟疫情调查处置进行描述和分析,指导继续开展消除疟疾工作。方法收集病例基本信息、流行病学资料、疫点自然情况、媒介监测情况等;收集本次疫情及既往间日疟病例全血,针对间日疟裂殖子表面蛋白基因进行套式PCR扩增并对产物测序分析。结果 2例病例为间日疟病例,主动病例搜索快速检测试验(RDT)检测病例周围居民467人,PCR检测103人,结果均为阴性;表面蛋白基因测序分析显示本次2例病例病原体间日疟裂殖子核酸序列高度同源,而与本省前几年本地病例则同源性不高,感染来源难以确定。结论 2例病例为本地感染病例,丹东地区存在传疟媒介,下一步应加大监测力度,继续调查感染来源;同时密切关注输入性病例,防止二代传播。Objective To summarize the investigation and management of two individual vivax malaria incidences in Dandong municipality of Liaoning province during August 2015 for promoting eradication of malaria in the area. Methods Information on demography and epidemiology of the two incidences, natural environment of the epidemic sites, and vector monitoring were collected. The whole blood samples for the two incidences and 11 previous vivax malaria cases were collected for amplification of surface protein gene of the vivax malaria's merozoite with nested PCR and the sequence analysis of the gene products. Results The two cases ( a 13-years old girl pupil and a 45-years old male electrician) were confirmed as vivax malaria. The results of rapid detection test (RDT) and PCR assay among 467 and 103 residents living around the houses of the two cases showed no positivity of merozoite surface protein. Sequence analysis of the surface protein gene of merozoite showed that the two cases were high homologous with each other and low homol- ogous with other local cases occurred a few years ago in Liaoning province. So the source of infection is difficult to deter- mine. Conclusion The two vivax malaria cases are local infection and local malaria vectors exist in Dandong municipal- ity. Surveillance needs to be strengthened to explore the source of the infections and close attention should be paid to imported case for the prevention of second generation incidence.
