椎旁肌间隙入路联合伤椎椎体内植骨对胸腰椎骨折患者疗效及并发症的影响

摘　　要：目的研究胸腰椎骨折行椎旁肌间隙入路和伤椎椎体内植骨联合治疗对患者疗效与并发症的影响。方法资料选取本院2015-03-2016-03收治的胸腰椎骨折106例患者予以回顾性地分析,根据临床所用不同治疗方案分成2组,将行常规入路治疗47例患者作为对照组,将行椎旁肌间隙入路和伤椎椎体内植骨联合治疗59例患者作为观察组,对两组疗效和并发症情况进行对比。结果观察组在两组治疗后3个月椎体前缘高度、Cobb角和治疗后24 h、1周VAS评分均改善基础上,其改善幅度比对照组显著(P<0.05);观察组总并发症率1.69%较对照组21.28%低(P<0.05)。结论胸腰椎骨折患者行椎旁肌间隙入路和伤椎椎体内植骨联合治疗效果显著,能够优化患者椎体前缘高度、Cobb角,且减轻腰背疼痛感与减少并发症发生,具有临床推广和应用价值。Objective To explore the effects and complications of paraspinal intermuscular approach combined with bone grafting in injured vertebra for the treatment of patients with thoracolumbar fractures. Methods 106 patients with thoracolumbar fractures who received treatment in our hospital from March 2015 to March 2016 were selected and divided into two groups based on the clinical treatment options. 47 patients in control group were given routine approach, and the other 59 patients in the observation group were given paraspinal intermuscular approach combined with bone grafting in injured vertebra. The efficacy and complications in the two groups were compared. Results After treatment, the anterior vertebral height, Cobb angle and the VAS scores 24 hours and one week after treatment were improved in two groups than before treatment, and the improvement in the observation group was more significant （P〈0.05）. The incidence rate of complications in the observation group was 1.69%, which was significantly lower than 21.28% in the control group （P〈0.05）. Conclusion For the treatment of thoracolumbar fractures, paraspinal intermuscular combined with bone grafting in injured vertebra has significant treatment effect, which can optimize patients＇ vertebral height and Cobb angle, alleviate back pains and reduce complications. Therefore, it is worthy to be popularized in clinical application.
