腹腔镜手术与开腹手术治疗胃十二指肠溃疡穿孔临床疗效比较

摘　　要：目的观察腹腔镜手术与开腹手术治疗胃十二指肠溃疡穿孔的临床疗效。方法选取医院外科就诊的胃十二指肠溃疡穿孔患者106例,随机分为腹腔镜组和开腹组,每组53例。腹腔镜组予以腹腔镜手术治疗,开腹组予以开腹手术治疗。观察2组手术时间、术中出血量、术后下床活动时间、肠蠕动恢复时间、镇痛剂使用率、术后住院时间及术后并发症发生率。结果腹腔镜组手术时间、术后下床活动时间、术中出血量、肠蠕动恢复时间、镇痛剂使用率和术后住院时间均显著优于开腹组(P<0.01);腹腔镜组切口出血、切口感染和肠梗阻发生率低于开腹组(P<0.05或P<0.01)。结论腹腔镜手术治疗胃十二指肠溃疡穿孔具有创伤小、恢复快、并发症少等优点,是治疗胃十二指肠溃疡穿孔更佳的术式。Objective To explore the clinical effect of laparoseopy and laparotomy operation in the treatment of gas- troduodenal ulcer perforation. Methods 106 cases of patients with gastroduodenal ulcer perforation,who were given the medial treatment , were divided into laparoscopy group （n = 53 ） and laparotomy group （n = 53 ） by table of random number, who were given repair of gastroduodenal ulcer perforation with laparoscopy and laparotomy operation respectively. Compared the operation time, amount of bleeding in the operation, postoperative out-of-bed activity time, recovery time of intestinal peristalsis, rate of use analgesics,length of stay and occurrence rate of complication of the two groups. Results The operation time, amount of bleeding in the operation, postoperative out-of-bed activity time, recovery time of intestinal peristalsis, rate of using analgesics, length of stay and occurrence rate of complication of laparoscopy group were significantly better than laparotomy group （ P 〈 0. 01 ） ;Incision bleeding,incision infection and intestinal obstruction rate in laparotomy group were higher than that in laparos- copy group （ P 〈 0. 05 or P 〈 0.01 ）. Conclusion Laparoscopy operation has the advantages of less trauma, faster recovery, less complications in the treatment of gastroduodenal ulcer perforation, which is a better surgical method for treating gastroduo- denal ulcer perforation.
