地佐辛和曲马多用于鼻内镜手术术后镇痛效果比较

摘　　要：目的观察地佐辛和曲马多用于鼻内镜手术术后镇痛效果和不良反应比较。方法将医院98例行鼻内镜手术的患者随机分为地佐辛组(49例)和曲马多组(49例),术后地佐辛组静脉镇痛泵配方:地佐辛20 mg+舒芬太尼50μg+昂丹司琼8 mg;曲马多组静脉镇痛泵配方:曲马多800 mg+舒芬太尼50μg+昂丹司琼8 mg,2组镇痛泵均加生理盐水稀释至200 ml。观察2组视觉模拟评分(VAS评分)、镇静评分(Ramsay评分)及不良反应发生情况。结果地佐辛组VAS评分在3 h、5 h、10 h优于曲马多组(P<0.05);地佐辛组Ramsay评分在术后5 h、10 h优于曲马多组(P<0.05);地佐辛组PCA按压次数在术后3 h、5 h优于曲马多组(P<0.05)。地佐辛组恶心、呕吐发生率低于曲马多组(P<0.05)。结论地佐辛用于鼻内镜手术术后镇痛方面较曲马多更为安全、有效,值得临床推广应用。Objective To explore and compare the postoperative analgesic effect and adverse reaction of dezocine and tramadol for nasal endoscopic surgery. Methods 98 patients underwent nasal endoscopic surgery were randomly divided into to dezocine group （49 cases） and tramadol group （49 cases）. Postoperative intravenous analgesia with dezocine formula： dezocine 20 mg ＋ sufentanil 50 p^g ＋ ondansetron 8 rag; Tramadol group intravenous analgesia formula： tramadol 800 mg ＋ sufentanil 50 pLg ＋ ondansetron 8 mg. The analgesic pump of 2 groups were diluted with normal saline to 200 ml. Visual ana- logue scale （VAS score）, sedation score （Ramsay score） and adverse reactions were observed in the 2 groups. Results The visual analogue scale （VSA） of dezocine group at 3 hours ,5 hours, 10 hours after operation was significantly more than tramad- ol group （P 〈0.05） ;The ramsay score of dezocine group at 5 h and 10 h after operation was significantly more than tramadol group （ P 〈 0.05 ）. The PCA pressing numbers of dezocine group at 3 h and 5 h after operation was significantly more than tra- madol group （P 〈 O. 05 ）. The incidence of nausea and vomiting of dezocine group was lower than tramadol group （P 〈 0. 05 ）. Conclusion Compared with tramadol group, Dezocine used for postoperative analgesia after nasal endoscopic surgery is safer and more effective, which is worthy of clinical popularization and application.
