摘 要:目的探讨糖皮质激素吸入治疗支气管哮喘的临床疗效及其对患者血清蛋白质水平的影响。方法选取2012年6月—2015年6月溧阳市人民医院收治的支气管哮喘患者167例,根据治疗方法不同分为研究组147例与对照组20例。两组患者均予以抗感染基础治疗,在此基础上研究组患者予以糖皮质激素吸入治疗,对照组予以抗炎平喘药物治疗,两组患者均连续治疗2周。比较两组患者肺功能指标[第1秒用力呼气容积占预计值百分比(FEV_1%)、呼气峰流速占预计值百分比(PEF%)及用力肺活量占预计值百分比(FVC%)]、白介素13(IL-13)和白介素4(IL-4)、血清蛋白质指标[热休克蛋白70(HSP70)、维生素D结合蛋白(VDBP)和嗜酸粒细胞趋化蛋白(Eotaxin)]及不良反应发生情况。结果治疗前两组患者FEV_1%、PEF%和FVC%比较,差异无统计学意义(P>0.05);治疗后研究组FEV_1%、PEF%和FVC%高于对照组(P<0.05)。治疗前两组患者IL-13、IL-4水平比较,差异无统计学意义(P>0.05);治疗后研究组患者IL-13低于对照组,IL-4水平高于对照组(P<0.05)。治疗前两组患者HSP70、Eotaxin、VDBP水平比较,差异无统计学意义(P>0.05);治疗后研究组患者HSP70、Eotaxin水平低于对照组,VDBP水平高于对照组(P<0.05)。两组患者均未发生严重不良反应。结论糖皮质激素吸入治疗支气管哮喘的临床效果确切,可改善患者血清蛋白质水平,且安全性高。Objective To investigate the clinical efficacy of glucocorticoid inhalation in the treatment of bronchial asthma and the impact on serum proteins. Methods A total of 167 bronchial asthma patients were selected from June 2012 to June 2015 in Liyang Municipal People 's Hospital,which were divided into study group( n = 147) and control group( n = 20).All patients were treated with anti-infective therapy,study group was given inhaled glucocorticoid treatment,control group was given anti-inflammatory and asthma medication,two groups of patients were treated for 2 weeks. The lung function index( FEV_1%,PEF%,FVC%),IL-13,IL-4 and serum proteins index( HSP70,VDBP,Eotaxin) and the incidence of adverse reactions were compared between the two groups. Results Before treatment, no significant differences of FEV_1%,PEF%,FVC% was found between the two groups( P〈0. 05); after treatment,FEV_1%,PEF% and FVC% of study group were higher than control group( P〈0. 05). Before treatment,no significant differences of the levels of IL-13 or IL-4 was found between the two groups( P〈0. 05); after treatment,study group of the level of IL-13 was lower than control group,and the level of IL-4 was higher than control group( P〈0. 05). Before treatment,no significant differences of the level of HSP70、Eotaxin、VDBP was found between the two groups( P〈0. 05); after treatment,the study group of the level of HSP70,Eotaxin were lower than control group,VDBP levels were higher than control group( P〈0. 05). There was no serious adverse reactions happened. Conclusion Glucocorticoid inhalation has a significantly clinical effect in the treatment of bronchial asthma,it can improve the serum proteins,and with high safety.
