叶下珠片联合拉米夫定治疗慢性乙肝的成本—效果分析

摘　　要：目的观察叶下珠片联合拉米夫定治疗慢性肝炎的治疗效果,并用经济学手段评价治疗成本—效果。方法将274例慢性乙肝患者随机分为2组各137例,对照组采用拉米夫定治疗,研究组采用叶下珠片联合拉米夫定治疗,比较2组患者治疗前后的肝功能指标,并计算整个治疗过程成本,比较2组患者的成本—效果。结果2组患者治疗后TBi L、DBi L、ALT、AST、AKP水平均明显优于治疗前,且研究组优于对照组,差异均有统计学意义(P<0.05)。成本—效果评价中,谷氨酸转氨酶复常率及乙肝病毒DNA转阴率C/E无明显差别(P>0.05),但达到相同效果的HBe Ag转阴率研究组明显优于对照组,且敏感度分析与上述研究结果无明显差异(P>0.05)。结论叶下珠片联合拉米夫定用于慢性乙肝能明显改善患者的肝功能状态,在以HBe Ag转阴率作为评价指标时,具有经济学效益,值得临床推广使用。Objective To observe the cost-effectiveness of leaf under the beads combined with lamivudine in Chronic hepatitis B. Methods 274 cases patients with chronic hepatitis B were randomly divided into research group and control group,each of 137 cases. Control group was treated by lamivudine. Research group was treated by Leaf under the beads com- bined with lamivudine. After treatment, Compared the liver function and cost-effectiveness of 2 groups. Results After treat- ment,The level of TBiL, DBiL, ALT, AST and AKP of 2 groups were lower than that before treatment, and the level of TBiL, DBiL, ALT, AST and AKP in research group were lower than that of control group, the difference was statistic significantly （P 〈 0.05）. In cost-effectiveness evaluation, the C/E of alanine aminotransferase recovery rate and HBV DNA negative rate was no difference （ P 〉 0.05 ）. When achieve the same effect, the HBeAg negative rate of research group was better than that of control group, and the sensitivity analysis and the results of the study was no significant difference（P 〉 O. 05 ）. Conclusion Leaf un- der the beads combined with lamivudine in chronic hepatitis B has an good effect, can improve the patient＇s liver functio. Set HBeAg negative conversion rate as an evaluation index with economic benefits, worthy of clinical application.
