腹腔镜原位脾切除术在外伤性脾切除术中的应用体会

摘　　要：目的探讨腹腔镜原位脾切除术在外伤性脾破裂脾切除术中的应用及体会。方法回顾性分析2例应用腹腔镜探查加腹腔镜下原位脾切除术治疗外伤性脾破裂患者的临床资料。结果 2例患者均在全腹腔镜下完成脾切除术,手术时间为102、113min,术中出血量为100、150m L,绕脐切口长度均约3cm,术后排气时间3、3~5d,住院时间7、9d。随访1~6个月恢复良好,无并发症发生。结论腹腔镜原位脾切除术治疗外伤性脾破裂是安全可行的。Objective To investigate the application experience of the treatment of laparoscopic splenectomy for patients with traumatic splenic rupture.Methods Two cases of traumatic splenic rupture underwent laparoscopic exploration and splenectomies were analyzed retrospectively.Results Laparoscopic splenectomy was successfully completed in two cases.The operation time were 102 and 113 min.The estimated intraoperative amount of blood loss was 100 and 150 m L.The incision around the navel was 3cm long.Postoperative exhaust time were 3 and 3.5days.Lengths of hospital stays were 7 and 9 days.From 1 to 6 months follow-up,the patients were healed well and no postoperative complication occurred.Conclusion It is safe and feasible for the treatment of laparoscopic splenectomy for patients with traumatic splenic rupture.
