两种腺样体切除术对小儿鼾症手术相关指标、ESS评分及血氧饱和度的影响

摘　　要：目的观察鼻动力和低温等离子两种腺样体切除术式对鼾症儿童手术相关指标、ESS评分及血氧饱和度的影响。方法选择我院2013年4月~2015年12月收治的鼾症儿童患者共170例,随机分为对照组（85例）和观察组（85例）,分别行鼻动力腺样体切除术和低温等离子腺样体切除术,术后比较两组患儿手术相关指标、近期疗效、手术前后QSQ评分、ESS评分、呼吸暂停低通气指数及夜间睡眠最低血氧饱和度水平等的变化差异。结果观察组患儿手术时间、术中出血量及症状缓解时间均显著优于对照组,差异具有显著性意义（P〈0.05）;两组患儿临床疗效比较差异无显著性意义（P〈0.05）;2组患儿手术前后QSQ和ESS评分、呼吸暂停低通气指数和夜间睡眠最低血氧饱和度水平均有明显改善,但观察组术后改善程度显著优于对照组（P〈0.05）。结论低温等离子腺样体切除术治疗小儿鼾症具有操作简便、术中创伤小及快速缓解相关症状等优势,可以更有效地改善患儿通气功能和机体缺氧状态。Objective To investigate the effects of two kinds of surgical procedures for adenoidectomy on operation-related indexes, ESS scores and blood oxygen saturation among children with snoring based on a clinical trial. Methods Included in this study were 170 children cases with snoring treated in our Hospital from Apr., 2013 to Dec., 2014. They were randomly divided into two groups, i.e. control group（CG） and observation group（OG）, with 85 cases in each. The cases in CG were treated by nasal power adenoid resection, and those in OG were treated by low-temperature plasma adenoid resection. To evaluate which was more appropriate to treat such a condition for children cases, a comparative analysis was made between the two groups to explore the differences in such indicators as operation-related indexes, short-term therapeutic effects, QSQ scores, ESS scores, apnea hypopnea index（AHI） and lowest level of blood oxygen saturation during sleeping period at night before and after the surgery by the end of hospitalization period. Results As shown from this trial, such indicators as operating duration, volume of bleeding during the operation and symptoms-relieving time were all significantly superior in OG to that of CG（P〈0.05）, while there was no significant difference in the therapeutic effects？between them（P〉0.05）. Moreover, there was significant improvement in scores of QSQ and ESS,AHI, and lowest level of blood oxygen saturation during sleeping period at night among cases of both groups as compared among the values pre-and post-operatively, but the improved range of measurements was significantly higher in those of OG than ones of CG（P〈0.05）. Conclusion Low-temperature plasma adenoid operating procedure is more effective in the treatment of children with snoring to improve ventilation function and anaerobic condition of patients,with such advantages as simple operation, much less injury to the body during the operation, and rapidly relieving related symptoms as well.
