腹主动脉阻断球囊植入术在凶险性前置胎盘剖宫产术中的应用

摘　　要：目的探讨腹主动脉阻断球囊植入术在凶险性前置胎盘剖宫产术中的应用效果。方法选取2012年11月至2014年9月四川省医学科学院收治的凶险性前置胎盘患者21例的临床资料进行分析。其中2013年9月至2014年9月在该院诊断为凶险性前置胎盘，并在剖宫产术前行腹主动脉阻断球囊植入术的13例患者为试验组：2012年11月至2013年8月在该院诊断为凶险性前置胎盘．采用剖宫产术终止妊娠，但术前未行腹主动脉阻断球囊植入术的8例患者为对照组，比较两组术中失血量、子宫切除情况及术后并发症。结果试验者患者13例，术中失血量200-2000mL。平均（873±129）mL，仅l例行次全子宫切除术，无介入术后并发症；对照组患者8例，术中失血量1500-5500mL，平均（2456±43）mL．2例行子宫切除术。结论剖宫产术中应用腹主动脉阻断球囊植入术治疗凶险性前置胎盘可有效减少术中出血量，最大限度地保留女性生育功能。Objective To investigate the effect of abdominal aorta balloon occlusion in cesarean section for pernicious placenta previa. Methods The clinical data in 21 cases of pernicious placenta previa in our hospital from November 2012 to September 2014 were selected and analyzed. Among them, 13 cases diagnosed as pernicious placenta previa in this hospital from September 2013 to September 2014 and performed the abdominal aorta balloon occlusion before cesarean section were taken as the experimental group;8 cases were diagnosed as pernicious placenta previa in this hospital from November 2012 to August 2013 and terminated the pregnancy by cesarean section without conducting preoperative abdominal aorta balloon occlusion were taken as the control group. The blood loss volume ,uterus resection situation and postoperative complications were compared between the two group. Results In 13 cases of the experimental group, the intraoperative blood loss amount was 200-2 000 mL, averaged （873±129）mL, only 1 cases performed subtotal hysterectomy without complications after intervetional operation;in 8 cases of the control group,the intraoperative blood loss amount was 1 500-5 500 mL, averaged （2 456±43）mL, 2 cases performed subtotal hysterectomy. Conclusion Applying abdominal aorta balloon occlusion in cesarean section for treating pernicious placenta previa can effective reduce the intraoperative blood loss and maximally reserve the female rel3roductive function.
