双层筋膜覆盖Medpor耳支架一期耳再造临床研究

摘　　要：目的观察双层筋膜覆盖Medpor耳支架一期耳再造术的临床效果。方法选择2013年1月~2016年5月在上海东方丽人医疗美容门诊部就诊的先天性小耳畸形269例,Medpor耳支架由美国PARK大学Porex外科医疗公司生产,雕刻好的外耳轮支架上覆盖游离筋膜瓣,翻转颞顶筋膜瓣覆盖整块支架,游离皮片覆盖筋膜瓣一期完成全耳廓再造术,术后2周,3个月,6个月及12个月随访,观察再造耳廓的成活情况及形态。结果 269例患者再造耳均成活,术后随访6~12个月未出现排异、过敏及其他不良反应。262例再造耳廓形态逼真,质感柔韧,立体感强,颅耳角与健耳对称,有3例出现支架外露,有4例因为瘢痕体质导致术后瘢痕增生,耳廓形态不佳。结论双层筋膜覆盖Medpor耳支架一期耳再造弥补了单层筋膜瓣覆盖Medpor支架植入质地偏硬的缺点,降低了外耳轮支架外露的缺点,是目前有效的应用Medpor耳支架一期耳再造的方法。Objective Stent exposed ear reconstruction is one of the most common and also the most difficult complications to repair. Medpor an ear scaffold To observe clinical results of covering a stent Medpor with double fascia in ear reconstruction sur- gery. Methods （1） Object： Select the 2013-01 / 2016-05 in Shanghai Oriental Beauty Medical Beauty clinic for treatment of 269 cases of microtia, preoperative surgical signed informed consent and informed consent to medical implants. （2）Main material： Med- por ear stent manufactured by PARK Porex Surgical Medical University. （3）Test Method： Free fascia covering the outer helix stent carving good, flip temporoparietal fascial flap block holder, free skin covering fascia complete a full assessment of auricular recon- struction. （4）Test： after surgery 2 weeks, 3 months, 6 months and 12 months follow-up, survival was observed auricular recon- struction and morphology. Results 269 cases of ear reconstruction patients survived were followed up for 6 to 12 months, aller- gies and other adverse reactions occurred. 269 cases of auricular reconstruction realistic shape, flexible texture, dimension, angular symmetry and auriculocephalic healthy ear. 4case because physical scars lead to postoperative scar poor ear shape. Conclusion Double fascia covering a stent Medpor ear ear reconstruction, only to make a single fascia covering Medpor texture more rigid stent shortcomings, but also significantly reduce the shortcomings exposed outer helix stent, it is an ideal application Medpor ear scaffold method of an ear reconstruction.
