心血管介入诊疗术患者心脏事件的高危因素分析及护理干预研究

摘　　要：目的观察分析心血管介入诊疗术患者心脏事件的高危因素分析以及探讨相应护理干预措施。方法选择2010年1月到2012年12月期间本院收治的行心血管介入诊疗术患者109例,根据患者手术开展前后是否出现心肌梗死或再梗死、心力衰竭、恶性心律失常中任一心脏事件分为高危组及正常组。将两组所收集的数据用单因素回归分析,再对危险因素进一步做多因素Logistic回归分析。结果本研究中的12项影响因素均属于心血管介入诊治术过程中或后引发心脏事件的危险因素;采用多因素Logistic回归分析,年龄≥60岁(RR=4.02)、高血脂(RR=3.98)、吸烟史≥2年(RR=4.36)、合并高血压(RR=5.4)、合并糖尿病(RR=7.61)、心绞痛病史(RR=5.34)、心肌梗死病史(RR=5.55)、脑血管意外病史(RR=4.21)、心力衰竭病史(RR=4.19)等因素属于高危因素。结论心血管介入诊疗术开展前后心脏事件发生的高危因素多且复杂,应引起高度重视,适当采取对症护理措施,以降低心脏事件发生率。Objective To explore the high risk factors of cardiac events in patients with cardiovascular interventional therapy and nursing intervention. Methoda From January 2010 to December 2015, 109 cases of patients with cardiovascular interventional diagnosis and treatment were divided into high risk group and normal group according to whether there was myocardial infarction or re-infarction, heart failure and malignant arrhythmia before and after operation. A single factor regression analysis was used to collect the data from the two groups, and then more factors Logistic regression analysis was used. Results The 12 influencing factors were the risk factors of cardiac events during the cardiovascular interventional therapy. Agea60 （RR=4.02）, high blood lipid （RR=3.98）, smoking history,2 years （RR=4.36）, combined hypertension （RR=5.4）, complicated with diabetes （RR=7.61）, history of angina pectoris （RR=5.34）, history of myocardial infarction （RR=5.55）, history of cerebrovascular accident （RR=4.21）, history of heart failure （RR=4.19） were the high risk factor. Conclusion The high risk factors of cardiac events before and after cardiovascular interventional diagnosis and treatment are many and complex, appropriate symptomatic care measures are used to reduce the incidence of cardiac events.
