髋臼双柱骨折手术入路的选择及疗效分析

摘　　要：目的髋臼双柱骨折手术入路的选择及疗效分析。方法收集我院2011年1月至2011年1月62例髋臼双柱骨折患者资料进行回顾性分析,将患者分为A组35例,B组27例。A组行髂腹股沟入路联合K-L入路,B组行单一髂腹股沟入路。采用改良的D'Aubigne评分系统进行评估关节功能,按Matta评定标准判定骨折复位质量,比较两组并发症发生率。结果 A组骨折复位优良率94.29%高于B组74.07%,差异明显(P<0.05)。A组关节功能优良率88.57%高于B组66.67%,有统计学意义(P<0.05)。A组术后并发症发生率2.86%低于B组37.04%,有统计学意义(P<0.05)。结论髂腹股沟入路联合K-L入路治疗髋臼双柱骨折效果较好,可提高骨折复位质量与关节功能优良率,降低并发症发生率。Objective To analyze the curative effect and select surgical approach for acetabular both-column fractures. Methods From Jan. 2011 to Jan. 2011, a total of 62patients with acetabular both-column fractures in our hospital were taken as the clinical research objects, and they were divided into A group （35 cases） and B group （27cases）. Patients in the A group accepted treatment through ilioinguinal approach combined K-L approach, and patients in the B group accepted treatment through ilioinguinal approach. The joint function was evaluated by the modified D＇Aubigne scoring system, the quality of fracture reduction was judged according to Matta criteria, and the incidence of complications was compared between the two groups. Results The excellent rate of fracture reduction in the A group was 94.29% which was significantly higher than that in the B group 74.07% （P〈0.05）. The excellent rate of joint function in the A group was 88.57% which was higher than that in the B group 66.67%, and their difference was statistically significant （P〈0.05）. The incidence of complications in the A group was 2.86% which was lower than that in the B group 37.04%, and their difference was statistically significant （P〈0.05）. Conclusion Ilioinguinal approach combined K-L approach, which has better effect in the treatment of acetabular both-column fractures, can improve the quality of fracture reduction and the excellent rate of joint function, and reduce the incidence of complications.
