探讨血清人附睾分泌蛋白4与癌胚抗原125联合检测在妇科盆腔肿块患者中的诊断价值

摘　　要：目的对妇科盆腔肿块患者进行血清人附睾分泌蛋白4(HE4)与癌胚抗原125(CA125)联合检测,以探讨其临床诊断价值。方法选取2014年6月-2015年6月在该院妇科诊断为盆腔肿块拟行手术的患者110例作为研究对象,其中卵巢癌患者38例(卵巢癌组),盆腔良性肿块患者72例(良性肿块组);另选取同期在该院进行健康体检的妇女50例作为对照组。分别采用酶联免疫吸附法与电化学发光法对3组患者的血清HE4与CA125含量进行测定并比较。结果与对照组比较,卵巢癌组与良性肿块组患者血清HE4及CA125水平均明显升高,差异具有统计学意义(P<0.05);与良性肿块组比较,卵巢癌组患者血清HE4及CA125水平均明显升高,差异均有统计学意义(P<0.05);HE4、CA125两项联合检测的敏感度为94.7%,与HE4单项检测、CA125单项检测的敏感度相比明显提高,差异具有统计学意义(P<0.05),而特异度及诊断符合率与单项检测比较,差异无统计学意义(P>0.05)。结论血清HE4及CA125是两种较为理想的盆腔肿块标志物,两者的水平测定对鉴别诊断妇科盆腔肿块良恶性疾病具有一定的作用,并且两者联合检测的敏感性明显高于单项检测,有助于提高盆腔肿块患者良恶性疾病的诊断价值。Objective To conduct combined detection of serum human epididymis protein 4（ HE4） and cancer antigen 125（ CA125）in patients with gynecological pelvic masses,explore the clinical diagnostic value.Methods A total of 110 patients diagnosed as gynecological pelvic masses and preparing for operation were selected from June 2014 to June 2015,then they were divided into ovarian cancer group（ 38 patients） and benign tumor group（ 72 patients）; 50 healthy women who received physical examination in the hospital during the same period were selected as control goup.ELISA and electrochemical luminescence method were used to detect serum levels of HE4 and CA125 in the three groups,and the results were compared.Results Compared with control group,the levels of serum HE4 and CA125 in ovarian cancer group and benign tumor group increased significantly（ P〈0.05）; compared with benign tumor group,the levels of serum HE4 and CA125 in ovarian cancer group increased significantly（ P〈0.05）; the sensitivity of joint detection of HE4 and CA125 was 94.7%,compared with single detection of HE4 or CA125,the sensitivity increased significantly（ P〈0.05）,but there was no statistically significant difference in specificity and diagnostic accordance rate between joint dection and single detection（ P〉0.05）.Conclusion Serum HE4 and CA125 are two ideal pelvic tumor markers.The detection of serum levels of the two markers has a certain value in differential diagnosis of gynecologic pelvic masses.The sensitivity of joint detection of the two markers is obviously higher than that of single detection,joint detection can improve the diagnostic value.
