自动听性脑干诱发电位在婴儿听力复筛中的应用调查

摘　　要：目的了解复筛未通过婴儿自动听性脑干诱发电位(AABR)筛查结果,分析影响AABR筛查结果的相关因素,比较AABR与瞬态声耳声发射(TEOAE)两种筛查方法的差异。方法选取2014年3月20日-2015年2月13日合肥市各县(区)妇幼保健所听力复筛未通过的361名婴儿为对象进行AABR筛查,采用卡方检验分析地区、性别、年龄及耳别对AABR筛查结果的影响。结果复筛未通过婴儿AABR筛查未通过率为44.6%。AABR未通过率经卡方检验,在性别、年龄、地区3个方面比较,差异均无统计学意义(P>0.05)。左耳未通过率高于右耳,且差异具有统计学意义(P<0.05)。AABR未通过率明显低于TEOAE未通过率,且两者差异具有统计学意义(P<0.05)。结论 AABR是一种有效的听力筛查方法,特别是对于复筛未通过婴儿进行后续的听力学评估之前,应用AABR筛查可有效降低假阳性率,提高筛查准确性,减轻部分家长不必要的精神负担和经济负担。Objective To understand the screening result of automated auditory brainstem response（ AABR） in the infants failing in secondary screening of infantile hearing,analyze the related factors,compare the difference between AABR and transient evoked otoacoustic emission（ TEOAE）.Methods AABR screening was conducted in 361 infants failing in secondary screening of infantile hearing in Maternal and Child Health Care Institution of Hefei from March 20 th,2014 to February 13 th,2015; chi- square test was used to analyze the effects of region,gender,age and ears on AABR screening result.Results The failing rate of AABR screening was 44.6%.There was no statistically significent difference in the failing rate of AABR screening among the infants living in different regions,with different genders and age（ P〈0.05）.The failing rate of AABR screening in left ears was statistically significently higher than that in right ears（ P〈0.05）.The failing rate of AABR screening was statistically significently lower than that of TEOAE screening（ P〈0.05）.Conclusion AABR is an effective hearing screening method,especially before subsequent audiological evaluation in the infants failing in secondary screening of infantile hearing,AABR can effectively reduce the false positive rate,improve screening accuracy,reduce unnecessary mental and economic burdens of some parents.
