摘 要:目的了解贵州省孕妇乙型肝炎病毒标志物(Hepatitis B Virus Markers,HBVM)与乙肝病毒脱氧核糖核酸(Hepatitis B Virus Deoxyribonucleic Acid,HBV-DNA)载量,探讨孕妇HBVM模式与HBV-DNA载量之间的关系。方法采用分层随机抽样的方法,随机抽取4个市州的9个县级医院作为调查单位,对调查单位住院分娩的孕妇血清采用酶联免疫吸附试验(ELISA)检测HBV标志物(Hepatisis B Virus Markers,HBVM),采用荧光定量聚合酶链反应(Fluorescence Quantitative Polymerase Chain Reaction,FQ-PCR)方法检测孕妇HBV-DNA载量。结果 534名孕妇感染HBV模式有19种,有299名HBV-DNA载量阳性占55.99%。HBs Ag、HBe Ag、Anti-HBc阳性(大三阳),HBs Ag、Anti-HBe、Anti-HBc阳性(小三阳)孕妇占总534名孕妇的65.10%。HBs Ag、HBe Ag双阳性的大三阳孕妇其HBV-DNA阳性检出率较高为95.42%,平均载量(7.06±1.27)Log IU/ml,与其他HBVM模式比较,差异有统计学意义(P<0.05)。结论孕妇HBVM检测结果中HBs Ag、HBe Ag为双阳性者,其HBV-DNA载量较高,该类人群是贵州省HBV母婴垂直传播的防控重点。Objective This study was to explore the hepatitis B virus markers (HBVM) and HBV-DNA levels among pregnant women in Guizhou province. Methods Stratified random sampling method was applied to take samples from pregnant women from 9 counties of 4 prefectures in Guizhou province by. Then, 5ml serum sample was taken from HBsAg-positive pregnant women for HBVM and HBV-DNA levels by enzyme-linked immunosorbent assay (ELISA) and Fluorescence Quantitative Polymerase Chain Reaction (FQ-PCR), respectively.The SPSS16.0 statistical software wasused for statistical analysis. Results 19 combinations of HBVM (HBsAg, Anti-HBs, Anti-HBc, HBeAg andAnti-HBe) were found among 534 pregnant women. 65.10% of the total were positive in the combination of HBsAg, HBeAg, Anti-HBc and in the combination of HBsAg, Anti- HBe, Anti-HBe. The total positive rate of HBV-DNA was 56% among 534 pregnant women. The positive rate of HBV-DNA and the average levels of HBV-DNA of HBsAg-positive and HBeAg-positive patients were 95.42% ,7.06±1.27Log IU/ml, respectively, higher than those of other HBVM. Conclusion The postive rate of HBV-DNA was the highest among the pregnant woman who were both HBsAg-positive and HBeAg-positive, thus they are the key population for the control of the maternal infant transmission of HBV in Guizhou province.
