摘 要:目的了解开封市健康人群麻疹抗体水平,探寻免疫预防重点人群。方法采用酶联免疫吸附试验(ELISA)对开封市健康人群进行麻疹Ig G抗体水平血清学检测,并对结果进行统计学分析。结果 1 878名调查对象麻疹抗体阳性率为75.99%(1 427/1 878);抗体达保护性水平的比率为42.39%(796/1878),抗体几何平均浓度(GMC)为330.31IU/L。各年龄组人群麻疹抗体阳性率和抗体GMC差异均有统计学意义(χ2=648.59,P<0.001;F=127.61,P<0.01)。其中,1岁以下婴儿抗体阳性率和GMC分别为45.00%(333/740)、103.02IU/L;20~29岁青年为93.79%(166/177)、645.43IU/L;1~4岁儿童GMC最高,为982.01IU/L;而40岁以上人群抗体阳性率最高,为98.48%。城镇人群麻疹抗体GMC(364.51IU/L)高于农村人群(293.37IU/L)。患过麻疹的人群抗体阳性率和GMC分别为99.39%、998.96IU/L均高于未患人群(67.68%、222.73IU/L)。既往接种过含麻疹成分疫苗的人群抗体阳性率和GMC分别为89.23%、591.07IU/L均高于未种人群(66.58%、218.14IU/L)。结论开封市患过麻疹、接种含麻疫苗的人群抗体水平高,而<1岁婴儿和20~29岁人群抗体水平较低,为麻疹防控重点人群,需通过综合性免疫策略来进一步提高其抗体水平。Objective The aim of this study was to analyze the measles immunity level in healthy populations in Kaifeng city. Methods A total of 1878 objects from six age groups in Kaifeng were detected by ELISA for measles IgG antibodies. We analyzed the data by t-test and X2-test. Results Positive rate of measles antibody was 75.99% (1427/1878) . The GMC of antibody was 330.31 IU/L. Positive rate and GMC of measles antibody were significantly different between population from different age groups (X2=648.59,/9〈0.001; F=127.61, P〈0.01). Antibody positive rate and GMC were lowest in the 〈1 year age group, which were respectively 45.00% (333/740) and 103.02 IU/L, and higher in the 20-29 year age group, which were respectively 93.79% (166/177) and 594.46 IU/L, and the rate was highest in the 40- year age group (98.48%), and the GMC highest in the 1-4 years age group (982.01 IU/L). Antibody GMC in urban population (293.37IU/L) was lower than that in rural population (364.51IU/L). Antibody positive rate and GMC in population with history of measles were respectively 99.39% and 998.96IU/L, which were higher than those population without. The rate and GMC in population with vaccination history were respectively 89.23% and 591.07IU/L, which were higher than those population without. Conclusion Among the healthy populations in Kaifeng, measles antibody level in the babies younger than one year old and the population aging between 20 and 29 years old were not enough to prevent outbreak and epidemic of measles. So, we should try our best to improve their measles antibody level.
