摘 要:目的了解宁夏自治区(宁夏)手足口病(HFMD)的病原谱,为今后防控提供科学依据。方法收集2010-2015年宁夏手足口网络实验室检测为肠道病毒阳性而肠道病毒71型(Enterovirus 71,EV71)和柯萨奇病毒A组16型(Coxsackievirus A16,CVA16)为阴性的标本,用RD细胞进行肠道病毒分离,分离的毒株用Real-time PCR筛检后,采用RT-PCR方法,使用兼并引物分别用A、B和C组肠道病毒进行VP1区扩增和核苷酸序列测定,测序结果编辑拼接后,用BLAST软件进行病毒型别鉴定。结果本研究共收集到标本773份,分离到毒株321株,分离率为41.62%(321/773),筛检出235株其他肠道病毒毒株,其中162株分别扩增出A组和B组肠道病毒目的基因,包括合并感染病例共鉴定出柯萨奇病毒A组(Coxsackievirus A,CVA)、柯萨奇病毒B组(Coxsackievirus B,CVB)和埃可病毒(Echovirus,ECHO)在内的17种型别的其他肠道病毒.。结果为65株CVA6,61株CVA10,12株CVA4,CVA 8和CVA14各3株,CVB5和ECHO(E)11各2株,CVA2、CVB1、CVB 3、CVB 4、E3、E 6、E 24各1株,合并感染7株。结论引起宁夏HFMD的病原谱多样,CVA6和CVA10是仅次于EV71和CVA16的主要病原体。Objective This study aims to analyze the etiological agents of hand, foot and mouth disease in Ningxia and to provide evidence for the prevention and control of this disease. Methods Specimens were collected from children infected with hand, foot and mouth disease during 2010-2015, which were identified as non-EV71, non-CVA16 enterovirus by real- time PCR method. RD cells were inoculated for virus isolation, and the VP1 complete nucleotide sequences of species A,B,C enterovirus genotypes were amplified by reverse transeriptase PCR (RT-PCR) and sequenced. Sequences were compared with the database of GenBank by the BLAST algorithm to identify the virus genotypes. Results A total of 321 strains were isolated from 773 specimens that were non-EV71, non-CVA16 enterovirus. The viral isolation rate was 41.62%. Among the 321 stains, 162 strains belonging to species A,B with a total of 17 enterovirus types were identifed, which included CVA6(65 strians), CVA10 (61 strians), CVA4 (12 strains), CVA8 (3 strains), CVA14 (3 strains), CVB5 (2 strains), ECHO (E)11 (2 strains), VA2 ( 1 strain), CVB 1 ( 1 strain), CVB3 ( 1 strain), CVB 4 ( 1 strain), E3 ( 1 strain), E6 ( 1 strain), and E24 ( 1 strain). In addition, there were 7 strains of co-infection detected. Conclusion The pathogenic spectrum of HFMD in Ningxia province is diverse. The CVA6 and CVA10 become HFMD main pathogens in addition to EV71 and CVA16. In addition, other enterovirus infections also cannot be ignored.
