超声检测下肢动脉硬化诊断冠心病的价值

摘　　要：目的探讨超声检测下肢动脉硬化诊断冠心病的价值。方法对确诊冠心病患者(研究组,86例)和同一时期收治的非冠心病患者(对照组,86例)行下肢血管超声检测,分析两组下肢动脉硬化情况及超声检测下肢动脉硬化对冠心病的诊断价值。结果研究组患者左、右股总动脉、股浅动脉近心段、股浅动脉远心段、腘动脉、胫后动脉、足背动脉血管内径均小于对照组(P<0.05)。研究组患者动脉中膜厚度大于对照组,斑块发生率高于对照组(P<0.05)。超声检测患者出现下肢动脉硬化诊断冠心病的灵敏度为72.09%,特异度为83.72%,阳性预测值为81.58%,阴性预测值为75.00%。结论下肢动脉硬化与冠心病关系密切。超声检测下肢动脉硬化对冠心病诊断有重要价值。Objective To explore the value of lower extremity arteriosclerosis detected by ultrasonography in the diagnosis of coronary heart disease（CHD）.Methods Vascular ultrasonography of both lower extremities was performed in 86 patients with CHD（group A）and 86 patients without CHD（group B）.The arteriosclerosis of lower extremities and diagnostic value of ultrasonography in CHD patients were analyzed.Results The blood vessel diameters of left and right stock total artery,proximal superficial femoral artery,distal superficial femoral artery,popliteal artery,posterior tibial artery and dorsalis pedis artery were smaller in group A than those in group B（P〈0.05）.The thickness of arterial tunica media was greater and incidence rate of plaque was higher in group A than those in group B（P〈0.05）.The sensitivity,specificity,positive predictive value and negative predictive value in the diagnosis of CHD were 72.09%,83.72%,81.58% and 75.00%,respectively,in the patients with arteriosclerosis of lower extremities detected by ultrasonography.Conclusion The arteriosclerosis of lower extremities is closely associated with CHD.The arteriosclerosis of lower extremities detected by ultrasonography has an important value in the diagnosis of CHD.
