摘 要:目的探讨儿童哮喘患者外周血Th17、Th22及Treg细胞及其细胞因子在儿童哮喘发病中的作用。方法收集本院2015年1月-2016年6月收治的儿童哮喘初发患者58例(BA初诊组),缓解患者45例(BA缓解组),以同期健康体检儿童30例作为正常对照组(HC组),采用流式细胞仪检测3组外周血Th17、Th22细胞及调节性T细胞频数。酶标法检测患者血浆中相关细胞因子IL-17、IL-21、IL-22及IL-35。结果 BA初诊组外周血Th17、Th22细胞比例及其相关细胞因子IL-17、IL-21、IL-22较HC组、BA缓解组均明显升高,差异均有统计学意义(P<0.05);BA初诊组外周血嗜酸性粒细胞绝对计数高于BA缓解组及HC组,差异均有统计学意义(P<0.05)。BA患儿嗜酸性粒细胞绝对计数与Th17呈正相关,而与Treg细胞呈负相关。结论 Th17、Th22和Treg细胞及相关细胞因子变化可能参与了儿童支气管哮喘的发病。Objective To investigate the roles of Th17,Th22,Treg cells and their related cytokines in the peripheral blood of patients with pediatric bronchial asthma. Methods 58 patients with incipient of bronchial asthma( BA incipient group) and 55 patients with remission group( BA remission group) were enrolled in our hospital during January 2015- June 2016,and 30 cases accepting physical examination were used as the normal control group( HC group). The frequencies of Th17 cells,Th22 cells and regulary T cells in the peripheral blood of the 3 groups were tested by flow cytometry instrument,and the related cytokines including IL- 17,IL- 21,22 and IL- 35 were tested by ELISA in plasma of patients with bronchial asthma. Results Concentrations of Th17,Th22 cells and their related cytokines( IL- 17,IL- 21 and IL- 22) were obviously higher than that in HC group and remission group,with the differences statistically significant( P〈0. 05); while the absolute count of eosinophils in remission group was significantly higher in the incipient group than HC group and BA remission group,with the differences statistically significant( P〈0. 05). The absolute counts of eosinophil in BA group were positively correlated with Th17,but negatively correlative with Treg. Conclusion The changes of Th17,Th22,Treg cells and related cytokines may be involved in the pathogenesis of bronchial asthma in children.
