摘 要:目的分析影响检验科微生物检验质量的主要因素以及病原菌的耐药性情况,为细菌感染性疾病的治疗提供参考。方法收集本院2014年7月-2015年7月进行微生物检测的560例患者资料,分析不同菌株的数量以及影响检测质量的因素。结果影响微生物检测质量的因素包括仪器设备质量、检验人员操作能力、标本采集/保存时间、检测方法等;共分离病原菌603株,革兰阴性菌比例最高,为312株(51.74%);其次为革兰阳性菌240株(39.80%),真菌51株(8.46%);大肠埃希菌对于喹诺酮类药物、头孢类药物耐药率超过50%,金黄色葡萄球菌对于喹诺酮类、红霉素类和β-内酰胺类药物的耐药性超过50%。结论了解影响微生物检测质量的因素,制定针对性的干预方式,提升检测效果;通过耐药性实验研究能够为临床治疗药物的选择提供依据。Objective To analyze the main factors affecting the quality of microbial inspection and the drug resistance of pathogenic bacteria,so as to provide reference for the treatment of bacterial infectious diseases. Methods The data of 560 patients who underwent microbial inspection from July to July 2007 were collected for analyzing the quantity of bacteria and the influence factors of detection quality. Results The influence factors of microbial detection quality included equipment quality,inspection staff operating capacity,specimen collection / storage time,detection methods; a total of 603 strains of pathogens were isolated,among which 312 strains of Gram negative bacteria took the highest proportion( 51. 74%),followed by 240 strains of Gram positive bacteria( 39. 80%) and 51 strains of Fungus( 8. 46%); the drug resistance rates of Escherichia coli to quinolones,cephalosporins were more than 50%,and that of Staphylococcus aureus to quinolones,erythromycin and beta- lactam drugs were more than 50%. Conclusion Understanding the factors affecting the quality of microbial detection and establishing the targeted intervention can improve the detection effect; the drug resistance experimental study can provide basis for the antibiotics selection in the clinical treatment.
