摘  要:目的采用婴幼儿喂养指数(ICFI)评价陕西省汉中市6~23月龄婴幼儿的喂养状况及其与体格发育之间的相关性。方法于2015年6月至2016年2月,按照多阶段分层整群随机抽样的原则,从西安交通大学医学院附属汉中3201医院及城固县、南郑县、洋县妇幼保健院儿保门诊随机抽取1 418名6~23月龄婴幼儿为调查对象,6~8、9~11、12~23月龄组分别为482、457、479名。采用自行设计的调查问卷收集婴幼儿基本情况及其喂养状况,父母家庭一般情况等信息,测量其身长与体重,进行ICFI评分(满分为16分),计算婴幼儿的身长别体重Z评分(WLZ)、年龄别体重Z评分(WAZ)、年龄别身长Z评分(LAZ),分析ICFI与WLZ、WAZ和LAZ的相关性。结果 1 418名婴幼儿ICFI得分为8.98±2.53;6~8、9~11及12~23月龄组ICFI得分为8.07±2.77、8.99±2.41和9.88±2.01,其合格率分别为32.2%、43.3%和60.1%,差异均有统计学意义(F=67.189,P<0.05;χ2=76.826,P<0.05)。农村与城市ICFI得分为8.70±2.51、9.20±2.52,农村喂养指数合格率为39.5%,城市喂养指数合格率为49.8%,差异均有统计学意义(t=-3.655,P<0.05;χ2=15.140,P<0.05)。ICFI得分不合格组的WAZ和LAZ均低于喂养指数合格组(t值分别为2.739、4.056,均P<0.05)。6~8、9~11及12~23月龄组ICFI得分均与LAZ呈正相关(r值分别为0.127、0.112、0.227,均P<0.05),6~8、12~23月龄组ICFI得分均与WAZ呈正相关(r值分别为0.179、0.161,均P<0.05),6~8月龄组ICFI得分与WLZ呈正相关(r值为0.152,P<0.05)。结论 ICFI在一定程度上反映了该婴幼儿群体的生长发育状况,可综合评价陕西省汉中市6~23月龄婴幼儿喂养状况。Objective To evaluate the correlation between feeding situation and physical development of infants and children aged 6- 23 months in Hanzhong City using infant and child feeding index( ICFI). Methods From June 2015 to February 2016,1 418 cases of infants aged 6 to 23 months from outpatients of child health care department of Hanzhong 3201 Hospital Affiliated to Xi'an Jiaotong University Health Science Center,and Maternal and Child Health Care Hospitals in Chenggu,Nanzheng,and Yangxian County were selected as investigation objects using multi-phase stratified cluster random sampling method,and they were divided to 6- 8 month old group( 482cases),9- 11 old month group( 457 cases),and 12- 23 month old group( 479 cases). Basic information and feeding situation of infants and general conditions of parents and families were collected using self-designed questionnaires. Infants' length and weight were measured and assessed with ICFI score( full score of 16). Infants' weight for length Z-score( WLZ),weight for age Z-score( WAZ) and length for age Z-score( LAZ) were calculated and correlation between ICFI score and WLZ,WAZ,LAZ was analyzed. Results Mean ICFI score of1 418 infants was 8. 98 ± 2. 53. Mean ICFI score of infants in 6- 8 month old group,9- 11 month old group and 12- 23 month old group was 8. 07 ± 2. 77,8. 99 ± 2. 41 and 9. 88 ± 2. 01,respectively,and pass rate was 32. 2%,43. 3% and 60. 1%,respectively. There were significant differences between different groups( F = 67. 189,P〈0. 05; χ2= 76. 826,P〈0. 05). ICFI score of rural and urban infants was8. 70 ± 2. 51 and 9. 20 ± 2. 52,respectively. Qualified rate of feeding index was 39. 5% and 49. 8% in rural infants and urban infants,respectively,and the difference had statistical significance( t =- 3. 655,P〈0. 05; χ2= 15. 140,P〈0. 05). WAZ and LAZ scores of ICFI-unqualified groups were both lower than those of ICFI-qualified groups( t value was 2. 739 and 4. 056,respectively,both P〈0. 05).ICFI scor
