摘 要:目的探讨孕期被动吸烟母体胎盘亮氨酸氨基肽酶(P-LAP)、白介素(IL-6)及IL-10在正常孕妇和早产孕妇血浆中的变化趋势,并分析其对早产的影响。方法选取175例早产孕妇(28~36+6周)为研究组(早产组),根据有无被动吸烟分为早产被动吸烟组(n=96)和早产非被动吸烟组(n=79);同时选取177例足月孕妇为对照组(足月组),同样根据妊娠期被动吸烟情况,分为足月被动吸烟组(n=78)和足月非被动吸烟组(n=99)。采用酶联免疫吸附法,测定以上各组孕妇在相应孕周时的血浆P-LAP、IL-6和IL-10浓度。结果早产被动吸烟组孕妇血浆P-LAP、IL-6和IL-10浓度与足月被动吸烟组比较差异有统计学意义(tP-LAP=3.678、tIL-6=3.865、tIL-10=2.114,均P<0.05),早产被动吸烟组孕妇血浆P-LAP、IL-6及IL-10浓度与早产非被动吸烟组比较差异有统计学意义(tP-LAP=4.479、tIL-6=4.556、tIL-10=2.202,均P<0.05);而足月被动吸烟组孕妇血浆P-LAP、IL-6及IL-10浓度与足月非被动吸烟组比较差异无统计学意义(tP-LAP=1.170、t_(IL-6)=1.722、t_(IL-10)=1.423,均P>0.05)。结论孕期被动吸烟可致孕妇P-LAP和IL-10水平降低,IL-6水平升高,引起早产分娩启动,可能是引起孕期被动吸烟孕妇早产发生的关键因素。Objective To study the changes of placental leucine aminopeptidase( P-LAP),interleukin 6( IL-6) and interleukin 10( IL-10) in mothers with passive smoking during pregnancy in full term pregnant women and pregnant women with preterm labor and to analyze their impact on preterm birth. Methods Altogether 175 cases of pregnant women with preterm labor( 28- 36+ 6weeks) were selected in observation group( preterm group) and further divided into preterm group with passive smoking( n = 96) and preterm group without passive smoking( n = 79) based on whether passive smoking existed during pregnancy. At the same time,177 cases of full term pregnant women were chosen in control group( full term group) and further divided into full term group with passive smoking( n = 78) and full term group without passive smoking( n = 99) according to situation of passive smoking during pregnancy. Plasma concentrations of P-LAP,IL-6 and IL-10 of above pregnant women in corresponding gestational weeks were detected using enzyme linked immunosorbent assay. Results Plasma concentrations of P-LAP,IL-6 and IL-10 in preterm group with passive smoking were significantly different from those in full term group with passive smoking( tP-LAP= 3. 678,tIL-6= 3. 865,tIL-10= 2. 114,all P〈0. 05). Plasma concentrations of P-LAP,IL-6 and IL-10 in preterm group with passive smoking were significantly different from those in preterm group without passive smoking( tP-LAP= 4. 479,tIL-6= 4. 556,tIL-10= 2. 202,all P〈0. 05). Plasma concentrations of P-LAP,IL-6 and IL-10 in full term group with passive smoking were not significantly different from those in full term group without passive smoking( tP-LAP= 1. 170,t_(IL-6)= 1. 722,t_(IL-10)= 1. 423,all P〈0. 05). Conclusion Passive smoking during pregnancy can cause drop of P-LAP and IL-10 levels and increase of IL-6 level,leading to start of preterm delivery,which is likely the key factor causing preterm delivery in pregnant women with passive smoking.
