宫腔镜联合屈螺酮炔雌醇治疗子宫内膜息肉的临床疗效观察

摘　　要：目的分析宫腔镜联合屈螺酮炔雌醇治疗子宫内膜息肉的临床疗效。方法 76例子宫内膜息肉患者,随机分为对照组和观察组,各38例。两组均行宫腔镜手术治疗,术后对照组不做任何处理,观察组术后第2天起给予屈螺酮炔雌醇片治疗,连用3个月。术后3个月统计比较两组患者不良反应,随访6个月后患者月经改善情况、子宫内膜厚度、疾病复发情况。结果 6个月随访时,观察组月经恢复正常率86.84%显著高于对照组的60.53%,复发率2.63%显著低于对照组的18.42%,子宫内膜厚度(6.39±1.13)mm明显小于对照组的(9.47±1.54)mm,差异有统计学意义(P<0.05)。两组患者术后3个月内均未出现大出血、宫腔粘连、宫腔感染、过度水化综合征等并发症。观察组用药初期轻度恶心1例,烦躁易怒2例,未经特殊处理自行缓解,不良反应发生率7.89%;对照组无不良反应,两组比较差异无统计学意义(P>0.05)。结论宫腔镜联合屈螺酮炔雌醇对子宫内膜息肉有显著的治疗价值,可降低疾病复发率,改善患者月经情况,对内膜增生加以抑制,值得推广。Objective To clinical effect of hysteroscopy combined with drospirenone and ethinylestradiol in the treatment of endometrial polyp. Methods A total of 76 endometrial polyp patients were randomly divided into control group and observation group, with 38 cases in each group. Both groups received hysteroscopic operation, and the control group received no processing after operation, but the observation group received drospirenone and ethinylestradiol tablets for treatment since 2nd day of postoperation for 3 months. Comparison were made on adverse reactions after 3 months of operation and menstrual improvement, endometrial thickness and recurrence rate of disease in 6 months of follow-up between two groups. Results The observation group had higher menstrual recovery normal rate as 86.84% in 6 months of follow-up than 60.53% in the control group, lower recurrence rate as 2.63% than 18.42% in the control group, lower endometrial thickness as（6.39±1.13）mm than（9.47±1.54）mm in the control group, and their differences had statistical significance（P〈0.05）. Both groups had no occurrence of complications in postoperative 3 months such as massive hemorrhage, intrauterine adhesion, enfection of uterus cavity and hyperhydration syndrome. The observation group had 1 mild nausea cases in early stage of drug use, 2 irritable cases, and all relieved spontaneously without special processing, with incidence of adverse reaction as 7.89%. The control group had no adverse reaction cases, and their difference had no statistically significant difference（P〉0.05）. Conclusion Hysteroscopy combined with drospirenone and ethinylestradiol shows significant therapeutic value in the treatment of endometrial polyp, which can reduce recurrence rate of disease, improve menstrual situation and restrain intimal hyperplasia, and worth promoting.
