摘  要:目的探讨磁共振扩散加权成像(DWI).对大鼠肝纤维化早期诊断及其分期的价值。方法CCl4法建立大鼠肝纤维化实验组,并行DWI检查,根据肝纤维化病理分期进行组间表观弥散系数(ADC)的比较。结果(1)随着肝纤维化病理分期的增加,DWI信号强度呈增加趋势,而ADC值逐渐减小。(2)b值为600s/mm^2和800s/mm^2时,对照组与实验组组间ADC值比较差异均有统计学意义(F=10.422,P〈0.001;F=16.637,P〈0.001);ADC值与肝纤维化分期呈负相关性(r=0.628,P〈O.001)。结论磁共振DWI的ADC值对肝纤维化早期诊断具有较高的价值,并具有较好的相关性;梯度因子b=800s/mm^2时ADC值对肝纤维化的分期更敏感。Objective To investigate the value of DWI in early diagnosis and staging of rats hepatic fibrosis. Methods The model of hepatic fibrosis by CCI4 was established on rats. DWI was performed in all rats and the ADC value was compared between rats of different pathological stagings. Results (1)As the pathological staging of hepatic fibrosis increases, the intensity of DWI rises while the ADC value declines. (2)The differences of ADC values among subgroups of different pathological stagings were statistically significant when b value was 600 s/ mm^2 (F=10.422, P 〈 0.001) or 800 s/mm^2 (F=16.637, P 〈 0.001). The ADC value was negatively correlated with pathological staging of hepatic fibrosis (r=-0.628, P 〈 0.001). Conclusion Hepatic fibrosis could be staged based on the value of ADC with good correlation when b value was 600 s/mm^2 or 800 s/mm^2, in which ADC was more sensitive to pathological staging of hepatic fibrosis when b was 800 s/mm^2 compared to 600 s/mm^2.
